Description
This is a combined synopsis/solicitation for commercial item(s) prepared in accordance with FAR12.6, as supplemented with additional information included in this notice. The United States Army Medical Research and Acquisition Activity (USAMRAA), Ft D etrick, MD intends to award an urgent and compelling contract, W81XWH-06-T-0315, for the following brand name consumable medical supplies: Fiberoptic MacIntosh Laryngoscope set, Manufacturer Dyna Med, catalog # AM704, quantity 12; Fiberoptic Miller Laryno scope Set with Handle Mil Blades, Manufacturer Dyna Med, catalog # AM701, quantity 187; Head-Master Head Immobilizer, Manufacturer Dyna Med, Catalog # IE017, Quantity 17; Master Cardiology Littman Stethoscope, Manufacturer Dyna Med, Catalog # DD799, Quanti ty 137; Spineboard Bak-Pak 72 X 16 X 2.25 Olive Green No Logo, Manufacturer Dyna Med , catalog # SB012, Quantity 550; Holster Kit Physician Compartmented Nylon, Manufacturer Galls, catalog # HK006, Quanity 372. These products are being ordered for the U.S. Medical Material Center in Europe. Many of these items are destined for units supporting Iraq and Afghanistan theaters. Resulting contract will be a one time award for the listed products. The Solicitation document and incorporated Provisions and Clauses are those in effect through Federal Acquisition Circular (FAC) 2005-07. The North American Industry Classification System (NAICS) code is 339112. The following Provisions and Clauses are incorporated and shall remain in full force in any resulting purcha se order: FAR Part 52.212-1 Instruction to Offerors - Commercial Items. FAR 52.212-2 EvaluationCommercial Items. This procurement will be awarded to that offer evaluated as low price, and technically acceptable. FAR 52.212-3 Officers Representations and Certification-Commercial Items, FAR 52.212-4 Contract Terms and Conditions - Commercial Items. Award will be made based on lowest price from responsive, responsible vendor. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statues or Execut ive Orders Applicable to Defense Acquisitions of Commercial Items, FAR 52.222-21 Prohibition of Segregated Facilities, FAR 52.222-26 Equal Opportunity, FAR 52.222-35 Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other El igible Veterans, FAR 52.222-36 Affirmative Action for Workers with Disabilities, FAR 52.222-19 Child Labor-Cooperation with Authorities and Remedies, FAR 52.233-1 Disputes, FAR 52.243-1 Changes-Fixed Price, FAR 52.232-36 Payment by Third Party, FAR 52.24 7-34 F.O.B. Destination, Medical Air Bridge, Savage, MD 20763 or Hanover MD 21076. Shipping shall be to the Medical Air Bridge, Savage, Maryland 20763, FAR 52.252-1 Solicitation Provisions Incorporated By Reference, FAR 52.252-2 Clauses Incorporated By R eference, DFARS 252.204-7004 Required Central Contractor Registration, and DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statues or Executive Orders Applicable to Defense Acquisitions of Commercial Items.
