- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) The purchase request number is F3M3C26116A100 and the solicitation is issued as a request for quotation (RFQ). The Wage Determination No.is: 1994-2393, Revision No.: 30; Date Of Revision: 05/23/2006. (iii) This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-11. (iv) This solicitation is 100% set-aside for small business concerns. The associated NAICS code is 541620, and the small business size standard is $6.5 Million. (v) This requirement consists of one Line Item: Complete Asbestos Survey. (vi) DESCRIPTION OF WORK: 1. SCOPE OF WORK. The contractor will provide the labor and materials required to perform the annual update to the baseline asbestos survey for 41 buildings. The contractor shall: (a) Review the existing baseline survey report and any other facility records which can be supplied by base personnel; (b) Visually inspect each facility to assess the current condition of documented Asbestos containing materials (ACM) (c) Utilize protocols established by the Asbestos Hazard Emergency Response Act (AHERA) (d) Provide a report, similar in format and structure to the baseline survey, documenting all findings. Baseline survey is available for review (e) Provide specific recommendations that address any needed ACM maintenance or removal, by building, location, and type NOTE: Inspectors must be accredited by the State of North Carolina to perform Asbestos Surveying. Inspectors shall place an adhesive sticker containing the month, year, and inspectors name at each location where asbestos is found. 1.1. DELIVERABLES The project deliverables are: (a) A final report, documenting the effort, findings, data, and recommendations as described in 1(d) and 2(e) above. (b) Four hard copies and a CD-ROM of the final report. 1.2 SCHEDULE: (a) The survey will be completed within 30 days from date of contract award. (b) The final report will be delivered not later than 90 days from date of contract award. WORKMANSHIP: All workmanship shall comply with standards and practices that are common to the trade. The contractor shall contact the POC when work is to begin and notify the POC when work is completed. CLEAN UP: The contractor is responsible for returning the work site and surrounding area to the condition in which they were found. NOTE TO CONTRACTOR: All work will be completed within 30 calendar days following the awarded date of this contract. Exceptions will be handled on a case by case basis. Changes and or modifications must have prior approval from the Contracting Office. WARRANTY: The contractor will guarantee all workmanship for a period of one year from completion of work. The contractor is responsible for any damages caused to government facilities. Note: All work will conform to all local, state, federal regulations, codes and requirements that are common to this trade. (vii) FOB shall be destination, and shipping should be included in the unit price. (viii) The provision at 52.212-1, Instructions to Offerors ? Commercial, applies to this solicitation. (ix) The provision at 52.212-2, Evaluation ? Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: Best Value. The contractor certifies that it can meet the delivery date by submitting a quotation. (x) The provision at 52.212-3, Offeror Representations and Certifications ? Commercial Items, applies to this solicitation. The contractor shall be registered in the Online Representations and Certifications Application ? ORCA ? at http://orca.bpn.gov/. (xi) The clause at 52.212-4, Contract Terms and Conditions ? Commercial Items, applies to this acquisition Paragraph (o) is changed to read: The contractor shall provide all standard commercial warranties to the government. (xii) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 are considered checked and are applicable to this acquisition: (13-19), (24), (29), (c1) and (c2). (xiii) Award can only be made to contractors registered in the Central Contractor Registration web page, http://www.ccr.gov. (xv) Commerce Business Daily numbered note 1 applies to this acquisition.
