Description
This is a combined synopsis/solicitation. Request for Quotation #RFQ 69D-402-06 entitled ?Locum Tenen Physician? is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. Multiple awards may be made for this requirement. Need for this requirement is immediate. Awards will be made to the vendor who can provide the first qualified candidates. This announcement incorporates provisions and clauses in effect through FAC 2005-07 and VAAR 97-10. NAICS Code 621111 applies for this procurement and the small business size standard is $9 million. This announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor?s proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor?s letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Price should be all-inclusive. Contractor?s price and technical proposal must contain the name, title and signature of person authorized to submit proposals/bids on behalf of the contractor. The contractor will provide the services of two (2) Board Certified/Board Eligible (within 4 years of completing residency), physician in Internal Medicine at the VA Medical Center, 325 East ?H? Street, Iron Mountain, MI. This position is to provide coverage for the Primary Care Clinic, on acute medical/surgical, NHCU, and Intensive Care Units. This physician will also be expected to function as the Medical Officer of the Day (MOD) on off tours. The primary care clinic hours of operation at Iron Mountain are from 8:00 a.m. to 4:30 p.m., Monday through Friday. Federal holidays are excluded from normal clinic hours of operation. Acute care consists of seventeen operating beds (12 med/surg. and 5 ICU). NHCU is a 36 bed combined rehab and nursing home care unit. Coverage will be provided from 8:00 a.m. to 4:30 p.m., Monday through Friday. Flexibility will be required in order to assure coverage for work areas. This is a full-time job opportunity. The Chief of Primary Care Service will provide the work assignments for this physician and will provide professional direction. The physician will be required to provide the Medical Officer of the Day Duties at the Iron Mountain VAMC. Medical Officer of the Day duties may be performed on weekends, holidays, or on weekdays. The tour may include working from 8:00 a.m. to 4:30 p.m., and from 4:30 p.m. to 8:00 a.m. the following morning. While serving as Medical Officer of the Day, the physician will be responsible for providing coverage as needed to all inpatient units and outpatients who present for care. In instances where the physician works as the Medical Officer of the Day, the physician will not be required to work the next day. The physician will be granted clinical privileges for the VA based on his/her specific expertise. This position is to support the direct patient care needs at the Medical Center and work will commence immediately upon completion of orientation. The physician will be required to work cooperatively with other physicians, advanced practice nurses, physician assistants, registered nurses, and a variety of allied health professionals to deliver comprehensive health care services to civilian and military patients. The medical center utilizes a computerized medical record system. The physician must be familiar with personal computers, have adequate typing skills to perform data entry (progress notes and physician orders), and be able to navigate through windows based computer programs. Training on the computerized system will be provided by the medical center. The physician must have good dictation skills and be able to utilize a centralized dictation system. Training on the dictation system will be provided by the medical center. The physician must provide proof that he/she has had a two-step tuberculosis skin test, testing is up-to-date, and documentation must be received prior to starting employment. If the physician has had a negative test within the past 12 months, a single step test will be done and again documentation needs to be provided prior to starting employment. SPECIFIC DUTIES AND RESPONSIBILITES: 1. The physician will follow and meet all medical center policies, performance measures, rules and regulations. 2. The physician is required to be certified in Basic Life Support (BLS) and to maintain the certification during their work at the medical center. 3. The physician is required to be Advanced Cardiac Life Support (ACLS) certified and to maintain the certification during their work at the medical center. 4. Function as a full team member of the interdisciplinary team, serving patients at the VAMC. 5. Provide history, physical examination, diagnosis, and initiation of medical treatment. 6. An outpatient progress note must be completed for each episode of care. 7. Initiate and facilitate treatment and transfer in emergency situations. 8. A discharge summary will be completed when a patient is released from inpatient care. Established criteria will be followed for the completeness and timeliness of the summary. 9. Follow-up office visits for existing medical problems and appropriate diagnostic tests and medication adjustments. 10. Referral of patients as needed to appropriate specialists, health care facilities, and coordination of home health services. 11. Document assessments and plan of care for each patient in accordance with policies and procedures. 12. Act as consultant in clinical practice. 13. Collaborate with other disciplines to ensure continuity of patient care. 14. Collaborate and act as a patient advocate. 15. Maintain knowledge of new and current clinical methods. Where the contract does not require JCAHO accreditation or other regulatory requirements regarding work competency, the contractor must perform the required work in accordance with JCAHO standards. The contractor is required to develop and maintain the following documents for each contractor employee working on the contract; credentials and qualifications for the job; a current competence assessment checklist (an assessment of knowledge, skills, abilities, and behaviors to perform a job correctly and skillfully; includes knowledge and skills required to provide care for certain patient populations, as appropriate); a current performance evaluation supporting ability of the contractor employee to successfully perform the work required in this solicitation and the listing of relevant continuing education for the last two years. The contractor will provide current copies of these records at the time of contract award and annually on the anniversary date of contract award to our VA COTR, or upon request, for each Contractor employee working on the contract. The contractor must also provide documentation that demonstrates that the contractor employee has a current physical that includes a current TB test and Hepatitis B vaccination. This includes: Curriculum vitae, current references, signed release of information from candidate, and statement that candidate does not have any physical or mental health condition that would adversely affect his/her ability to carry out assigned duties, and additional credentialing and privileging forms.
