- Title: Tissue Spectrometer This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this Request for Quotation is V3047-0260. This solicitation document incorporates provisions and clauses in effect through FAC 05-11. This solicitation is Unrestricted. The associated NAICS Number is 334510. The FSC Code is 66. VA Special Services has a requirement for the following item(s): Quantity of (1) InSpectra Tissue Spectrometer ? Model 325, Part Number 5001569; Quantity of (1) Inspectra Tissue Spectrometer Model 325 User and Service Manual, Part Number 5000110; Quantity of (1) InSpectra Optical Cable ? 15mm, Part Number 5001442; Quantity of (1) InSpectra Optical Cable ? 25mm, Part Number 5001440; Quantity of (1) InSpectra Calibrator with 4 Shields per Calibrator, Part Number 5003754; Quantity of (1) InSpectra System Check, Part Number 5004767; Quantity of (1) InSpectra OptoLink RS232 Optical Converter, Part Number 5000290; Quantity of (1) Data Fiber, Part Number 5000195; Quantity of (1) InSpectra Software Version 2.0; Part Number 5005376; Quantity of (2) InSpectra United States Power Cord, Part Number 5000076; Quantity of (1 Carton) InSpectra Calibrator with 4 Shields per Calibrator; Part Number 5003754. The Manufacturer of this equipment is Hutchinson Technology. Brand Name or Equal. Delivery, which shall occur within 60 calendar days of contract award, shall be FOB DESTINATION to Lackland AFB, TX 78236. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. The Federal Contractor Program requires that any contractor receiving a contract from the Federal Government in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
