Description
The Department of Veterans Affairs, Network 22 Logistics Office, has a requirement for a consolidated contract for all labor, materials, equipment, and transportation necessary for on-site procedures and technical support of Mobile Lithotripsy Services at VA San Diego, VA Greater Los Angeles, VA Long Beach and VA Loma Linda, all located within Southern California and VA Southern Nevada. The procedures may vary for the different hospitals in regards to use of ElectroHydraulic or Electromagnetic Extracorporeal Shock Wave Lithotripsy and in either case, a Holmium Laser machine is required for each site-appointment as well in case the need arises for immediate follow-up of the ESWL procedure. There also may be the need for Holmium Laser treatment as a stand-alone procedure for Ureterscopy or Percutaneous Nephroscopy Lithotripsy (PCNL) for certain hospitals. The selected vendor or vendors must own, operate, and provide the technical operational support for each of the three machines. The intended contract will be for a base year with the Governments right to extend the contract in one-year increments for a possibility of up to four option years. This notice is open to qualified sources of small businesses relative to 2002 NAICS Code 621111 (size standard $9.0 million) of a Request for Proposal (RFP) solicitation for Mobile Lithotripsy Services which is now posted herein as a 100% Small Business set-aside. All offerors are advised to thoroughly review the entire document, paying special attention to all required documentation required to be submitted in the proposal. In addition to the fill-in sections of the proposal, additional documents are required in the Work Statement and Provisions sections of the RFP. If any vendor prefers to have a hard copy sent to them as opposed to downloading and printing a copy from this site, a written request must be sent to via one of the methods and addresses shown below.
