- The Contracting Department, Fleet and Industrial Supply Center (FISC), Norfolk, VA, 23511-3392 intends to issue a modification on contract number N00189-05-D-0018 awarded to AMO USA, INC. The contract is a firm fixed price contract issued on a sole source basis utilizing procedures under FAR Subpart 13.5, Test Program for Certain Commercial Items. The contractor provides 300 packages of Excimer Laser High Mytopic Treatment Cards to include one (1) Card (10 Treatments), two (2) Enhancement Cards, and two (2) Prevue Lens Cards for Naval Medical Center, 54 Lewis Minor Street, Portsmouth, VA 23708-2297. These cards will be used with Government owned STAR S4 IR Laser. The period of performance is 01 Oct 2005 through 30 Sep 2006. This modification will add three hundred (300) packages of Excimer Laser High Mytopic Treatment Cards to the existing contract.
