- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This is a notice of the Governments intent to solicit, negoti ate and award a sole source (Note 22) contract to Carl Zeiss Surgical, Inc. for an OPMI PRO ERGO Microscope Ceiling Mount System (2 each), an optional Xenon Light for the Pro Ergo (1 each), an Angled Optics w/ Beam Splitter (1 each), and a Binocular Rotato r Adapter (1 each). The Dental Clinic (Endodontics) at Blanchfield Army Community Hospital, Fort Campbell, KY 42223 has a requirement for the OPMI PRO ERGO Microscope Ceiling Mount System inclusive of external and internal parts as specified on the Equipme nt Specification Data Sheet. Requested date of delivery is 30 September 2006. This equipment will be utilized as a dental surgery device. This acquisition is issued as a 100% Small Business Set-A-Side Open Market Procurement to obtain the maximum amount o f competition from all interested vendors. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors cons idered. The North American Industry Classification System Code (NAICS) for each item is 339112, size standard is 500 employees. The following Federal Acquisition Regulation (FAR) provisions are applicable to this solicitation: FAR 52.212-1, 52.212-2, 52. 212-3, 52.212-4, 52.212-5, 52.219-6, and 52.232-33, DFARS: 252.232-7003 & 252.204-7004.
