- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number is W91YTZ-06-T-0124. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect thro ugh Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is 339112. This requirement is 100% set-aside for Small Business. This procurement is being conducted under FAR subpart 13, where simplified acquisitions procedures apply and the conduct of the procurement will be in accordance with Parts 12 or 15 of the FAR or some combination thereof, as stated in FAR 13.003(g)(2). The Womack Army Medical Center, Fort Bragg, NC, has a requirement f or the following items: CLIN 0001 Unfors X-ray Measurement Meter Kit; Quantity: 3; Unit of issue: each. The Unfors X-ray Measurement Meter Kit shall include,at a minimum: Unfors R/F MAM detector; Unfors Xi CT Detector; Unfors Xi Bluetooth serial adapter, Unfors Xi HVL kit. Offerors shall include unit price and total price with their quotes for CLIN 0001. The manufacturer is Unfors Instruments Inc. Similar equipment will not be accepted or considered for award. Quotes shall be submitted in a separate sheet of paper in accordance with the instructions on this solicitation. The Government anticipates awarding a single award for this requirement. The items shall be delivered FOB destination to Womack Army Medical Center, Bldg 4-2817 Reilly Rd., Fort Bragg, NC 28310, within 4 weeks after receipt of order. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial, apply to this acquisition. Addenda to FAR 52.212-1: Paragraph (f) of Far clause 52.212-1, Instructions to Offerors, does not apply to Request for Quotations (RFQ).
