- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the attachment. This announcement constitutes the Request for Quotation; and a separate request for quotation will not be issued. The United States Attorney Office for the Southern District of West Virginia has a requirement for Grand Jury and Deposition Reporting Services. It is anticipated that a Blanket Purchase Agreement will be awarded for a sixty (60) month period. Offerors must have the capability to establish and office within a fifty (50) mile radius from locations listed in the Statement of Work. This notice is prepared in accordance with FAR Part 13, Simplified Acquisition Procedures, NAICS with size standard M. THIS IS A TOTAL SMALL BUSINESS SET-ASIDE PROCUREMENT. QUOTES ARE BEING REQUESTED. The solicitation number is RFQ-EOA88-6-0201. See ATTACHMENT for the following additional information: Instructions for completion of solicitation package, Schedule of Commercial Services and Prices, Statement of Work and the following clauses 52.212-4 ? Contract Terms and Conditions-Commercial Item? and 52.212-5 ? Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items. Offers or quotations will be evaluated in accordance with FAR 13.106-2 and other evaluation factors contained in the solicitation ?Evaluation Criteria?. All questions regarding the RFQ shall be submitted via email to Patricia.Willett@usdoj.gov on or before 12:00 noon est., July 17, 2006. Quotes are due on Friday, August 11, 2006 at 4:00 p.m., Eastern Standard Time. LATE Quotations will not be accepted. Quotes shall be sent via e-mail, courier, hand delivered or faxed to The Department of Justice, Executive Office for the U.S. Attorneys, 600 E Street, N.W., Suite 2400, Acquisition, Facilities and Support Services, Washington, D.C. 20530, Attn: Patricia Willett. You may fax your quote to (202) 616-6651 or e-mail to Patricia.Willett@usdoj.gov. Offerors shall ensure that the faxed quotes are LEGIBLE (min. 12pt font).
