Description
VA National Acquisition Center will issue Request for Proposal (RFP) VA-797-06-RP-0054 for the Department of Veterans Affairs (VA), Department of Defense (DOD), Bureau of Prisons (BOP) and Indian Health Services (IHS) for Carbidopa/Levodopa Tablets. The estimated annual usage of Carbidopa 25mg/Levodopa100mg Tablets, SA is 42,378 bottles of in bottles of 100 each and 109,695 bottles Carbidopa 50mg/Levodopa 200mg Tablets, SA in bottles of 100 each. The contract period will be for one year plus four one-year option periods. The contract items will be distributed through the VA and DOD's respective Pharmaceutical Prime Vendors (PPV). Offerors must provide bar code labeling which must state the exact name (name that appears on label) by which the drug will be supplied and must have a unique NDC number. The first 5 numbers must identify the offeror. Bar Code Validation is the process of ensuring the data on a bar code label received from suppliers meet minimum requirements for font, readability, data integrity, and completeness. All bar codes must be in a linear format and conform to all UCC/EAN standards. This means the bar code can be any linear bar code symbiology, such as UCC/EAN-128, RSS, or UPC (if the UPC contains the NDC). Subject to Good Manufacturing Practices (GMPs). Bar code must provide the expiration date and lot number. Offerors must provide bar coded product that meets these minimum requirements. In addition, tablets must be compatible with automated dispensing units (Baxter ATC Canisters, Optifil, etc.). Glass bottles are not acceptable. All bottles of 100s must have child resistant closure. If the offeror is not a manufacturer, a signed agreement between the offeror and the manufacturer must be submitted with the offer and remain in effect for the duration of the contract period including the option years.
