Description
The USAF Air Force Academy Contracting Office has a requirement to purchase from a sole source intended for Medtronic Xomed, 6743 South Point Dr, N; Jacksonville, Florida 32216-0980, Phone 800-874-5797; for a NIM Response 2.0. Medtronic is the only known manufacture capable of providing this equipment for the USAF Academy Surgical Clinic. Currently the surgical staff have been trained and are proficient in the use of the NIM Response 2.0. The Commercial items is in accordance with the format in Subpart 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals being requested and a written solicitation will not be issued. Solicitation number: FA7000-06-T-NIM20 issued as a request for quotation (RFQ). A firm-fixed price type contract will result from this solicitation. Solicitation, amendments and other miscellaneous documents will be posted to FedBizOpps (http://www.eps.gov). It is the responsibility of each offeror to review the web page for notice of amendments, updates or changes to current information. This acquisition will be conducted using Full and Open competition procedures. Description: CLIN 0001: Nerves Monitor 1 EA., NIM Response 2.0, 82-52600; CLIN 0002: 2 EA., copies of Operator and Maintenance/Ser vice Manuals. CLIN 0003: 1 EA., Medtronic Xomed is willing to discount the previously used unit at $9,510.00 appling the it to cost of the new replacement. Warranty and service statement shall be provided. The place of delivery and acceptance will be USAF Academy, 10th Medical Group 4102 Pinion Drive, Academy, Colorado 80840. The contractor will provide the earliest date of delivery. All Offerors must quote FOB Destination, USAF Academy, 10th Medical Group 4102 Pinion Drive, Academy, Colorado 80840. The following Clauses and Provisions apply to this requirement: FAR 52.204-7. Additional contract requirement(s) or terms and conditions: Award will be made to the lowest priced technically acceptable responsible offeror. This acquisition will be solicited in accordance with (IAW) Far Part 12-Acquisition of Commercial items. Small disadvantage women are encourage to participate. This competitive acquisition is unrestricted.
