- Description
- Luke AFB is looking for a contractor to perform monthly inspections of approximately eight elevators and quarterly inspections of two wheel chair lifts. Replacement parts and emergency calls may be required. The performance period is 1 October 2006 through 30 September 2007. This acquisition is contingent upon availability of funds. The anticipated award date is 1 October 2006 and it will be a firm fixed price purchase order. The acquisition is full and open competition. The North American Industry classification System (NAICS) code is 81131, size standard is $5 mil. The Statement of Work, (SOW), is attached. Please provide pricing for CLIN 0001: Main base preventive maintenance, 12 months ________ and total amount __________; CLIN 0002: Hospital preventive maintenance, 12 months __________and total amount ___________; CLIN 0003: Repair for main base (routine, emergency, and parts greater than $400.00) Not to Exceed Amount is $3,400.00; CLIN 0004: Repair for hospital (routine, emergency, and parts greater than $400.00) Not to Exceed $400.00; CLIN 0005: Semi-annual certification for main base by licensed inspector, 2 each __________and total amount _____________; CLIN 0006: Semi annual inspection for hospital by licensed inspector, 2 each ___________and total amount __________; CLIN 0007: Chair lift preventive maintenance, 4 quarters _____________and total amount ________________.
