Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, streamlined procedures for evaluation and solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be posted but emailed upon request. This solicitation, FA3022-06-Q-0008, is a Request for Quotation using simplified acquisition procedures in accordance with FAR Subpart 13.5. The associated North American Industry Classification System (NAICS) for this procurement is 621111 and the small business size is $8.5M. This solicitation is being advertised for full and open competition. The Government requires 1 Full Time Equivalent (FTE) for this Performance Work Statement (PWS). The Contractor shall provide general medical provider services required for Government beneficiaries as outlined in this Performance Work Statement. The Contractor shall provide care at the Columbus AFB Clinic, also referred to as the "medical treatment facility" (MTF) herein. Position must be filled by one of the following medical provider categories (listed in order of preference): Any healthcare provider with Flight Medicine experience at Columbus Air Force Base, Medical Doctor (MD)/Doctor of Osteopathic (DO) with Family Practice experience, Family Nurse Practitioner or Physician Assistant with Family Practice experience. Performance shall be according to the requirements contained in this (PWS), and professional standards of the Joint Commission on Accreditation of Healthcare Organization (JCAHO). LICENSURE/CERTIFICATION. IAW paragraph 3.1, AFI 44-119, Clinical Performance Improvement, 4 Jun 2001, non-personal services contract employees must maintain an active license or authorizing document from the state of Mississippi. Assignment to a position not involving direct patient care within or outside an MTF does not eliminate the requirement for license or authorizing document. The performance period will consist of one year, 5 Jul 2006- 4 Jul 2007. All interested parties must be registered with the Central Contractor Registration (CCR) as prescribed in DFARS 252.204-7004 and 252.232-7009. CCR can be accessed at www.ccr.gov or by calling 1-888-227-2423. The CCR registration must be completed before award can be made. All interested parties must also be registered with Wide Area Work Flow (WAWF) in accordance with IAW DFARS 252.232-7003, Electronic Submission of Payment Requests (Dec 2003) WAWF can be accessed at https://wawf.eb.mil.
