- **The purpose of Amendment #5 is to accomplish the following: 1. Extend the due date from 19 May 2006 to 26 May 2006 in accordance with the statment posted on 15 May 2006 2. Upload a REVISED Attachment #1 (Bid Schedule) to the solicitation dated 18 May 2006 3. Upload a REVISED Attachment #2 (Statement of Work) to the solicitation dated 18 May 2006. Please note that an additional Questions/Answers (#2) document has been uploaded to the Federal Business Opportunities website dated 18 May 2006.** Altus Air Force Base has a requirement for the following commercial item: contractor is to set up and maintain a Contractor Operated Civil Engineer Supply Store (COCESS) in government facilities and will be responsible for furnishing Civil Engineer-related supplies for use on Altus Air Force Base, Oklahoma. Typical categories of supplies will include: electrical, plumbing, carpentry, paint, HVAC, hardware, and related supplies for repair, maintenance, or remodeling of new or existing facilities/property. Contract performance is for a basic period of 01 May 2006 through 30 September 2006 with four (4) one-year option periods. This Request for Quotation (RFQ) is a competitive best value acquisition utilizing simplified procedures authorized by FAR 13.5, Test Program for Certain Commercial Items. All offerors doing business with the government MUST register in the DOD Central Contractor Registry (CCR) database prior to receipt of submittals. Failure to register in the CCR database will render your firm ineligible for review and possible award. CCR registration may be accomplished online at: http://www.ccr.gov. Additional information concerning CCR registration may be obtained by calling: 1-888-227-2423. All firms must also be registered with the Online Representations and Certifications Application (ORCA) website prior to receipt of submittals in order to be considered for award. This may be accomplished at: http://orca.bpn.gov/. The single source of entry for all documentation pertaining to this acquisition will be on the government procurement website at the following web address: http://www.fedbizopps.gov. All interested parties are responsible for checking this website to ensure they have the most current information (i.e. amendments issued, changes to the Statement of Work, etc). Faxed requests for a solicitation package will not be accepted.
