Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 1.This solicitation is issued as a Request for Quotation (RFQ) from the Veteran?s Administration, Denver Distribution Center (DDC). It is numbered 791-15-06. 2.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-09. 3.This procurement is a total set-aside for Small Business Concerns. NAICS Code: 339113 Small Business Size Standard: 500 employees Set-Aside: Small Business Set-Aside, offers will not be accepted from large business concerns. 4.Line Item 1: Incubator, 185M Large Chamber, Airborne Life Support System. Required quantity is 4 (four) each. 5.Description: Infant transport incubator. Circulates heated air. Item must be able to use ?E? size oxygen tanks secured under the unit. International Biomedical Model 185M, or equal. 6.Date(s) and place(s) of delivery and acceptance and FOB point. DELIVERY INFORMATION Delivery schedule: 60 days after receipt of order, and is defined as the date material is due in, not the shipping date. FOB point is DESTINATION unless otherwise specified. Delivery Address: 882nd TRSS/TRSL (Medical Logistics) 939 Missile Road, Bldg 1919, Ste 2 Sheppard AFB, TX 76311-2245 Mark for: TCN: FM300460747106 (Must be included on all packaging) SHIPPING REQUIREMENTS The commercially accepted shipping containers must be clearly marked to indicate the contractor?s name and the following: A.Name and complete address of the contractor B. VA Purchase Order Number C. Government Bill of Lading Number covering the shipment (if any) D. If ?Mark for:? information is specified, then this information MUST appear on all containers. E. A commercial packing list, including the description of material shipped, name of item, item number, quantity, number of containers, and package number (if any), should be included. F. If more than one order is shipped per container, each order shall be packed in a separate container or package and must contain an individual packing list, as specified above, for that order. 7.The provision at 52.212-1, Instructions to Offerors?Commercial, applies to this acquisition and the following addenda apply: ADDENDA TO 52.212-1: Offers may be submitted via e-mail to ray.blomquist@va.gov, or fax to (303)914-5420. Products samples are not required. However, see clause FAR 52.211-6 and VAAR 852.211-77 for documentation requirements if an ?or equal? item is offered. Multiple offers will be accepted. Offers for different manufacturers are considered multiple offers. 8.The provision at 52.212-2, Evaluation?Commercial Items, will be used for this procurement. The following addenda to this provision are applicable: ADDENDA TO 52.212-2: The Government will award an order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers (in order of importance): Technical (conformity to specifications), price, past performance. Technical and past performance, when combined, is significantly more important than cost or price. The importance of price will increase if offerors are considered essentially equal in terms of technical value. 9.Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications?Commercial Items, with its offer.
