- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation includes applicable provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-09. This synopsis/solicitation is identified as FDA-SOL-06-00218. This requirement shall be set aside for small businesses. Scope of Work: Provide and Install VTC Polycom VSX 7000E units at three remote site locations. The contractor shall provide the following items: (1) Polycom ? VSX 7000E, (2) 1 Year of service support, (3) NT3 Triple NT1 for ISDN Termination, (4) VSX Quad BRI Module. The contractor shall install the VTC system, test and tune the entire system, provide training to the client on site, and provide one set of manufacturer?s instruction manuals with one year of service support. The three sites are at the following locations: (1) Dauphin Island, 1 Iberville Drive, Dauphin Island, AL 36528; (2) Moffett Center, 6502 S Archer Road, Summit-Argo, IL 60501; (3) MOD 1, 8301 Muirkirk Road, Laurel, MD 20708. Our remote sites should be compatible to interface with the current systems located in the Wiley building and NIH Radvision Bridge. This is a Request for Quotation (RFQ). Any interested party should submit a statement of capabilities along with a price quotation to determine if the requirements of this synopsis can be met. The provision at FAR 52.212-1, Instructions to Offerors ? Commercial Items, applies to this solicitation. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. The clause at 52.212-4, Contract Terms and Conditions ? Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes Or Executive Orders ? Commercial Items, applies to this acquisition. The following FAR clauses cited are applicable: FAR 52.222-3, FAR 52.222-19, FAR 52.222-21, FAR 52.222-26, FAR 52.225-1, FAR 52.225-13, FAR 52.232-33. Clauses and provisions are incorporated by reference and apply to this acquisition.
