- Description
- The Phoenix Area Indian Health Service (PAIHS) is soliciting proposals from qualified contractors to provide all necessary resources, personnel, supplies, equipment, facilities, and other items and expertise necessary in providing medically necessary ground ambulance service for Native American patients within the Phoenix Metropolitan and surrounding areas, including Salt River Pima-Maricopa Indian Reservation, Scottsdale, Arizona. The period of performance shall be for one year (base year) with four (4) one-year options. Solicitation package will be available March 17, 2006 and may be obtained by written request to e-mail address matilda.jones@ihs.gov or to facsimile number (602) 364-5030. No telephone request will be accepted. Cite RFP 975. Closing date for receipt of proposals is April 17, 2006, 4:30 PM, Arizona time.
