This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect throu gh Federal Acquisition Circular 2005-07. The North American Industry Classification System Code (NAICS) for this procurement is 334510. The Moncrief Army Community Hospital, Fort Jackson, South Carolina 29207-5720, has a requirement for ONE VIDEONYSTAGMO GRAPHY SYSTEM THAT INCLUDES DESK TOP COMPUTER WITH 156X DVD ROM; 15in LCD FLAT SCREEN; VNS CAMERA GOGGLES; PCL DELAGIO VIDEO CAPTURE BOARD; LCD PROJECTOR FOR OCULAR STIMULATION; VNG ANALYSIS SOFTWARE FOR OCULAR AND VESTIBULAR TESTING; FOOTSWITCH; AIR STIMU LATOR; TWO USER/SERVICE MANUALS. Please include shipping cost and date of shipment. The Government anticipates awarding a single award for this requirement. The items shall be delivered to the Moncrief Army Community Hospital, USA MEDDAC PBO, ATTN: Chan dra Walker, Bldg 2500, Marion Street, Fort Jackson, South Carolina 29207 as soon as possible after receipt of order.
