- The Veterans Integrated Service Network (VISN) i9 is soliciting to procure Optometrist Services for the VA Medical Center, 800 Zorn Avenue, Louisville, KY 40206. The Contractor(s) shall provide properly licensed Optometrist(s) to provide refraction, and routine eye examinations in accordance with the contract Statement of Work. This is an unrestricted commercial contract for a base year and two one-year options to renew. The facility(ies) where the services are to be provided must be accessible to Veteran beneficiaries assigned to the Patient Service Area (PSA) of the VAMC Louisville. Contracts to multiple contractors may be awarded if in the best interest of the Government. The NAICS code is 621320 and a small business side standard of $6.0 million applies. Solicitations will be issued o/a 9/12/05.
