Description
COMMERCIAL COMBINED SYNOPSIS/SOLICITATION (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number R6-14-06-39047 is issued as a Request for Quotations (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-8 (iv) This is a small business set-aside with an associated NAICS code of 442210 and small business size standard of 500 employees. (v) Description of Line Items The following are this solicitation?s list of contract line item number(s) and items, quantities and units of measure: 1. Removal of old carpet and installation of new carpet in the North Fork John Day Ranger District main office first floor, steps to the second floor and both landings, quantity 1, unit of measure lump sum 2. Installation of new carpet in the 16-person bunkhouse at North Fork John Day Ranger District, quantity 1, unit of measure lump sum. 3. Option Item for removal and installation of new carpet on the second floor of the main of the office, quantity 1, unit of measure lump sum. (vi) PERFORMANCE WORK STATEMENT: a. Description of Work: ? Remove existing and install new carpeting in the North Fork John Day Ranger District Office, Building 2015. Includes all areas currently covered with carpeting on the first floor, steps to the second floor and both landings. Work will also include sealing all holes and penetrations in the sub-floor with caulk or insulation. ? Prepare the new sub-floor and install new carpeting in 16-person bunkhouse (needs to be started on first) ? Optional item for installing carpet on the second floor of main office. (vii) The estimated start date is 5/1/2006 and the require completion date is 6/15/2006, if option item 3 is awarded the required completion date is 6/30/2006. (ix) The provision ?52.212-2, Evaluation-Commercial Items?, applies to this acquisition and the specific evaluation criteria to be included in paragraph (a) of that provision, are as follows: Price only. Description Of Work In Terms Of Results Or Outcomes Install new modular carpet tiles in both the North Fork John Day Ranger District office and the new 16 person bunkhouse. Item 1. Ukiah Office ? remove and dispose of the old carpeting in the first floor of the office, the stairs, hallways and landings. Supply and install approximately 545 square yards of new modular carpet tiles in all areas where carpet was removed. Work includes all labor and materials, metal or vinyl reducers and related materials, including the moving of furniture. Item 2. Ukiah Bunkhouse ? clean and prepare subfloor, supply and install approximately 160 square yards of modular carpet tile per specifications and drawings. Work includes all labor and materials, metal or vinyl reducers and related materials. Item 3. An optional bid item for the completion of the second floor will be made. If funds are available, the second floor will be awarded also ? approximately 286 square yards. Work includes all labor and materials, metal or vinyl reducers and related materials, including the moving of furniture. PART 1 - GENERAL SUMMARY Remove old carpet, provide carpet tile and floor preparation. Work includes all labor and material necessary to complete the job, including moving of furniture. SUBMITTALS Product Data: Submit manufacturer's product data and installation instructions for each material and product used. Samples: Submit manufacturers color samples upon award of contract. QUALITY ASSURANCE Comply with governing codes and regulations. Provide products of acceptable manufacturers which have been in satisfactory use in similar service for three years. Use experienced installers. Deliver, handle, and store materials in accordance with manufacturer's instructions. Performance: Fire performance meeting requirements of building code and local authorities. PART 2 - PRODUCTS MATERIALS Manufacturers: Blue Ridge Touchstone, Lees Commercial Carpets Faculty IV, or approved equal. Carpet Tile: Material: As selected. Installation Method: Glue-down. Auxiliary Materials: Edge guards. Adhesives, cements and fasteners. Leveling compound. PART 3 - EXECUTION INSTALLATION Prepare surfaces and install materials in accordance with manufacturer's instructions and approved submittals. Clean, patch, and level substrate. Once the old carpeting in the office has been removed, seal all cracks, joints, penetrations, holes, etc. in the subfloor, that would allow air movement between the crawl space and first floor, with silicone caulk, or foam insulation. Further, seal the annular space between the subfloor and floor duct vents with silicone caulk. Install materials in proper relation with adjacent construction and with uniform appearance. Coordinate with work of other sections. This work includes moving of all furniture, desks, cabinets, etc. Furniture shall be replaced in the same spot from which it was moved. Install edge guards and reducer strips as required; clean and protect.
