FA8601-06-T-0071 BRAND NAME OR EQUAL PAOLI FURNITURE ***************************** AMENDMENT 0003 IS BEING POSTED TO THE FEDBIZOPS TODAY (3/28/06) TO CHANGE THE SET-ASIDE DESIGNATION FOR THIS REQUIREMENT FROM 100% SMALL BUSINESS SET-ASIDE TO FULL AND OPEN. THE CLOSING DATE/TIME REMAINS UNCHANGED AT 4:00 P.M. EST, 31 MARCH 2006. ***************************** This is a combined synopsis/solicitation for commercial items as supplemented with additional information included in this notice. This item is prepared in accordance with (IAW) the format in FAR Subpart 12.6, and this announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This is a Brand Name or Equal Acquisition. In accordance with (IAW) FAR 52.211-6 Brand Name or Equal, Wright Patterson Air Force Base has a requirement to purchase and installation of office furniture for the Commander?s Suite Renovation at HQ AFMC/CC in Bldg 262, Area C, Reference Atch. 1 Parts Listing Paoli. Quote all Paoli or none and F.O.B. Destination. The contractor shall provide purchasing action and complete operational installation of the furniture for AFRL/CC, Wright Patterson Air Force Base, OH. Required delivery date is forty five (45) days after the award date. The furniture and installation must be completely operational NO LATER THAN FORTY FIVE DAYS (45) AFTER THE AWARD DATE.. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-07 Effective 02 Feb 2006 and & Class Deviation 2005-o0001. Solicitation FA8601-06-T-0071 is a Request for Quotations (RFQ). North American Industry Classification System (NAICS) code is 337211 and the size standard is 500 employees. This solicitation is FULL AND OPEN (REFERENCE AMENDMENT 0003). The clause at FAR 52.237-1, Site Visit, applies to this acquisition A Site Visit is planned for 24 Mar 2006 at 1:00 P.M. at Hope Hotel, Wright-Patterson AFB, OH in front of Packy?s which is a restaurant in the Hope Hotel. Transportation will be provided. Transportation will remain at the Hope Hotel until 1:15 P.M. after which time we will depart for the Site visit. The Hope Hotel is located in Area C before entry into Gate 12C. You must submit the full names and addresses of all attendees (not to exceed two per company) to Barbara P. Jemmott, Contract Specialist, at 937-522-4566 or e-mail: barbara.jemmott@wpafb.af.mil by 22 Mar 2006 NO LATER THAN 2:00 P.M. EST. Moreover the following information is required for each attendee: full name and address of the company being represented. This information must be provided in order to ensure access to the Base for the site visit. Valid and current picture identification is required for this Site Visit and must be visible at all times while attending this site Visit. Offerors are requested to submit questions as a result of the Site Visit to the point of contact noted above not later than 27 Mar 2006, 4:00 P.M. Information provided by this Site Visit shall not qualify the terms and conditions of the combined synopsis/ solicitation. Terms of the combined synopsis/solicitaton remain unchanged unless amended in writing by the Contracting Officer. If an amendment is issued, normal procedures relating to the acknowledgement and receipt of synopsis/solicitation amendments shall apply. A record of the Site Visit shall be made by the Government and furnished to all prospective offerors. The record shall include minutes of the meeting, including questions (on a non-attribution basis) and answers, if applicable. All work shall be in accordance with the Specifications, which can be accessed from the World Wide Web address at the Federal Business Opportunities (FBO) site: http://www.fbo.gov/. Solicitation provision at Federal Acquisition Regulation (FAR) 52.212-1 (Jan 2004), Instructions to Offerors ? Commercial is hereby incorporated by reference. The following paragraphs (a) thru (c) are supplemental instructions (Addendum to Provision FAR 52.212-1) to offerors for development of quotations. (a) Purpose: These instructions prescribe the format of quotation and describe the approach for the development and presentation of quotation data. They are designed to ensure the submission of information essential to the understanding and comprehensive validation of quotations. The instructions permit the inclusion of any additional data or information an offeror deems pertinent. Offerors are cautioned to follow the detailed instructions fully and carefully, as the government intends to make an award based on initial offers received, without discussions of such offers. (1) Completed Request For Quotations Quotations shall be submitted NO LATER THAN 31 MAR 2006 AT 4:00 PM EST to 88 CONS/PKBA, Attention: Barbara P. Jemmott, Contract Specialist Area C, Building 1, Room 109, 1940 Allbrook Drive, Suite 3, Wright Patterson AFB, OH 45433-5309 (2) Past Performance: In the quotation, the offeror shall submit a Present/Past Performance Reference List citing the contract number, name, phone number and fax number of Point of Contact (POC), dollar value of contract and period of performance on three recent and relevant contracts for the same or similar items. The OFFEROR will submit a copy of the Present/Past Performance Questionnaire (attached to this website) to each of the three references with instructions to send completed questionnaires to the Contracting Specialist NO LATER THAN 31 Mar 2006 at 4:00 P.M. EST, the address label shall reflect the following information: Solicitation Number 06T0071, ?To be delivered unopened to the Contract Specialist, Barbara P. The government will use information submitted by the offeror as well as other sources to conduct its survey. IMPORTANT NOTICE TO CONTRACTORS: Any award resulting from this solicitation will include DFARS Clause 252.232-7003, Electronic Submission of Payment Requests. Section 1008 of the National Defense Authorization Act of Fiscal Year 2001 requires any claims for payments (invoices) under DoD contract to be submitted in electronic form. Wide Area Workflow-Receipt and Acceptance (WAWF- RA) is the DoD system of choice for implementing this statutory requirement. Use of the basic system is at no cost to the contractor.
