- L.A. Job Corps is requesting bids for two dental service contracts and one dental hygienist service contract, by which the general dental services will delivered to approximately 735 Job Corps students. Job Corps is a federally funded training and education program for youth ages 16-24 years and who meet certain other criteria. The dental services will be provided in our facility located at 1106 South Broadway Ave., Los Angeles, CA 90015. 1. Dentist is required to: a) Work 40 hours every two weeks. b) Must be licensed in the State of California. c) Must provide Professional Liability Insurance and Controlled Substance Registration Certificate. d) The dental services will include general practice: 1) Preventive dental health education. 2) 24 Hour emergency on call coverage including weekends. 3) Oral examination, to include four bite-wing X-rays. 4) Restorative and prophylactic treatment. 5) A diagnosis and treatment plan for each student within two weeks after completion of routine dental examination, including a listing of the teeth involved and specific services to be rendered. 2. Dental Hygienist is required to: a) Work a maximum of 8 days per month, 2 days per week. b) Must be licensed in the State of California. c) Must provide Professional Liability Insurance. d) The dental hygienist service will include general hygienist practice. e) Preventive dental helth education which promote good oral hygiene techniques. The terms of the contract for the dental services will be for two years commencing on May 1, 2006 and continuing until April 30, 2008 with the option to extend these contracts's period for up to three years in successive one year increments. YWCA of Greater Los Angeles operates the Los Angeles Job Corps Center under contract with the U.S. Department of Labor, Employment and Training Administration. The successful bidders must also comply with all Fderal Acquisition Regulation clauses and be compliant under all related federal statutory requirements and regulations.
