Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-06-T-0041. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effe ct through Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is 339111. This requirement is 100% total Small Business Set a-side. The Martin Army Community Hospital, Fort Benning, GA, has a requirement for following Medical Equipment: CLIN 0001 - TABLE,UROLOGY, C-ARM, MODEL# 058-800, 115 VAC, Dims: 84.5in l x 25.5in w (214.6 x 64.8cm) with OR accessory rails. Tabletop: 53in l x 21in w (134.6 x 53.3 cm): 84in l (213.4cm) with ext ension. Lightweight Extension: 26.5in l x 21in w (67.3 x 53.3 cm.). Radiolucent Extension: 31in l x 25.5in w (78.7 x 64.8 cm) with OR accessory rails. Radiolucent Area: 54in l x 17.5in w (137.2 x 44.5 cm). Tabletop Material: Three fiberesin sections-each with individual 2in mattress. Table Sections: Four Sections: Adjustable Fowler back 0 degrees to 80 degrees, abdominal section with removable mattress pad, radiolucent removable extension and a light weight non-imaging transfer extension for transferring patients on and off the table. Mattress: individual 2in thick (5 cm) one for each section. Controls: Head Control: actuates all motions, Foot Control: actuates all motions. Displays actual table top position (all axes) in real time and provides menu s and prompts for other functions. Joystick Controller: for motorized XY positioning.
