- This is combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announce constitutes the only solicitation; quotes are bein g requested and a written solicitation will not be issued. The solicitation number is W91YTV-06-T-4748. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect throu gh Federal Acquisition Circular 2005-07. The North American Industry Classification System Code (NAICS) for this procurement is 339112. The Eisenhower Army Medical Center, Fort Gordon Georgia 30904, has a requirement for 32 Assorted Items that include th e following: Traverse Guidewire, Voyager OTW Balloon, Mini-Vision RX Stent, and Vision RX Stent. Please contact Mr. Raymond E. Hulse to inquire about specific sizes and quantities. Please include shipping cost and date of shipment. The Government antic ipates awarding a single award for this requirement. The items shall be delivered to the Eisenhower Army Medical Center, Building 300 Warehouse, ATTN: SGT Oliveri, Fort Gordon, Georgia 30905 as soon as possible after receipt of order.
