- This reopens the presolicitation notice and changes the response date to May 1, 2006. The USDA Forest Service, Region 2, has a requirement services to perform routine Dental Services at Pine Knot Civilian Conservation Center, Job Corps Rd. Pine Knot, McCreary County, KY 42635. This request for proposal is designated as a Small Business set-aside. NAICS code: 621210. Size standard 6 million. This contract will be awarded as a Firm Fixed Price Contract with a Base Year Period and Four (4) Option Year Periods. The Contractor shall provide all personnel, transportation, supplies, materials and other resources (except for Government-furnished property or facilities described herein) necessary to complete the work described.
