- The following are the answers to all of the questions that were submitted by 10 Mar 06. DENTAL HYGIENTIST SOLICITATION #FA4610-06-T-4010 QUESTIONS AND ANSWERS 1. What is the name and phone number of the current incumbent? There is no current incumbent, but the last awarded contract went to Virginia Aldaco, 4512 Cynbalaria, Santa Maria, Ca, 93455-3700. The contract number was FA4610-04-P-0114 and it was awarded for $37,500.00. 2. What is the current bill rate? See question 1. I am unable to break it down any further. 3. What is the price for the previous or current contract for these services? The previous contract was awarded for $37,500.00 4. Are they planning to pursue this project again? Yes. 5. Since the unit pricing is based on a monthly price, how would the contractor calculate the billing if the provider missed a day of work? Please see the amendment to the solicitation. The unit has been changed from months to hours. 6. Is it possible to change the units to hours? See queston 5. 7. Is this a new or existing requirement? The old contract ended in Sept 05. Currently there is no existing contract for these services. 8. Bidders list or list of persons or planholders list. No bidders list will be issues. 9. Main differences between old statement of work and the current one. The requirements stayed the same. 10. Can we respond to this bid by fax or e-mail? Per the instructions in this soliciation, you can respond by fax or email. 11. Yes, this requirement is subject to the wage determination rates. A copy of the wage determination will be attached to this solicitation. 13. What is the price or contract budget? There is no contract budget. The Government will evaluate all quotes based on what will be most advantageous to the Government with total price being the major consideration. All other terms and conditions remain the same. An amendment to the Solicitation and the Wage Determination have been attached.
