Description
This is an amendment to the combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA4419-06-Q-0301. This is a 100% Small Business Set-Aside. The point of contact for this RFQ is Carl See at (580) 481-7321 or Charles Peterson at (580) 481-5838. Provide 50 Chemical Protective Overgarments, jackets and pants in the sizes listed, Non Flame Resistant; suggested source is LANX Fabric Systems http://www.lanxfabrics.com/CPOPartNumbers.htm; (866) 731-5186 in the following quantities and sizes: 5 each LANX Part Number CPO-NFR-MS; 17 each LANX Part Number CPO-NFR-MR; 7 each LANX Part Number CPO-NFR-ML; 13 each CPO-NFR-LR; 7 each LANX Part Number CPO-NFR-LL; 1 each CPO-NFR-XL; Coats and trousers shall be silk-screened "TRNG ONLY" in 2 1/2 inch white letters using plastic based resin ink. The coat will be marked on the back, 5 inches below the collar and centered between the shoulders. The trousers will be marked on the right leg, centered 1 inch below the pocket along the vertical axis; Pricing to include FOB Destination for delivery to Altus AFB, OK 73523-5132. Vendors wishing to submit a like item other than from the recommended source must send manufacturer technical data sheet along with the proposal for evaluation by the technical team. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-07. The following FAR Clauses and Provisions apply to this acquisition: Provision 52.212-1, Instructions to Offerors-Commercial Items; Provision 52.212-3, Offeror Representations and Certifications- Commercial Items; Clause 52.212-4, Contracts Terms and Conditions- Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items the following subparagraphs apply: (a), (b)(5), (b)(14-20), (b)(24)(i)(ii), (b)(26), (b)(31), and (e); DFARS Provision 252.204-7004, Required Central Contractor Registration (CCR); DFARS Clause 252.232-7003, Electronic Submission of Payment Request. Registration is required in the CCR database in order to receive a Government contract. DFARS Provision 252.212-7000, Offeror Representations and Certifications. Offerors shall include a completed copy of FAR provision 52.212-3 and DFARS Provision 252.212-7000 with their quote (Note: On-line Representations and Certifications Application (ORCA) has been implemented in FAR Clause 52.212-3 effective 01 Jan 05. ORCA enables vendors to submit and update their representations and certifications on-line. ORCA is part of the Business Partner Network (BPN) and is located at http://orca.bpn.gov. The ORCA site contains an ORCA Application Handbook and an ORCA Quick Reference Guide). DFARS Clause 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. 5352.201-9101 Ombudsman (AUG 2005) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (c) If resolution cannot be made by the contracting officer, concerned parties may contact the Center/MAJCOM ombudsmen. Concerns, issues, disagreements, and recommendations that cannot be resolved at the MAJCOM/DRU level, may be brought by the concerned party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (703) 588-7004, facsimile number (703) 588-1067.(d) The ombudsman has no authority to render a decision that binds the agency.(e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer. (End of clause). The applicable North American Industry Classification System (NAICS) Code is 339999. The Small Business Size Standard is 500 employees. Award will be based upon the offer representing the Best Value to the Government. Paper copies of the Request for Quotation will not be made available. NOTE: Quotations must be received by 20 March 2006, 4:00PM CST; late quotations will not be considered for award.
