- Under the authority of Public Law 104-262 and 38 USC 513, the contractor agrees to provide Multi-Lumen Catheter Kits and Percutaneous Sheath Introducer Kits, in accordance with pricing, terms and conditions stated in the solicitation, to the Department of Veterans Affairs (VA) participating medical facilities in VISN 12 (northeastern Illinois, Upper Peninsula of Michigan and Wisconsin). The kits should contain all safety devices in accordance with and to become compliant with CDC recommendations and OSHA requirements. The solicitation contains a base year with four one-year options. The NAICS code and small business size standard is 339112, 500 employees. Electronic version and hard copy of solicitation should be available on or about March 13, 2006. Electronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov/. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs.
