- This is not a solicitation. This is a Sources Sought Announcement to research the market's capability to provide CUSTOM SURGICAL PACKS to the VA Ann Arbor Healthcare System in Ann Arbor, MI. Please respond to Kristina Harden by 3:30 PM EST Friday, March 17, 2006 if your business is capable of providing Custom Surgical Packs meeting the following makeup and requirements. When responding, please indicate whether or not your business currently holds a Federal Supply Schedule (FSS) contract for the supplies requested below. If your business does currently hold an FSS contract, please provide the contract number. Also provide your business size status (i.e. large or small) and any business certifications your business holds (i.e. HubZone, Women-owned, Veteran-owned, Service Disabled-Veteran-Owned, 8A, Disadvantaged, etc.). CUSTOM SURGICAL PAKCS ARE TO INCLUDE THE FOLLOWING COMPONENTS: 1) 1/2 inch tubing for oxygenator; 2) Trillium Affinity 541T; 3) 3/8 inch tubing for oxygenator; 4) 3/8 inch x 1/2 inch tubing connectors, for oxygenator; 5) 3/8 inch Y connector with luer lock; 6) Affinity 40 micron filter; 7) Pre-bypass filter; 8) Myotherm cardioplegia set (1/4 inch tubing, heat exchanger, 3/16 inch tubing, pressure monitoring set, and 2 stopcocks); 9) Quick prime line, 1/4 inch tubing with 2 spikes; 10)1/4 inch tubing for vent and suckers; 11)One way valve in vent and suckers; 12) Gas filter; 13) One way valve line; 14) DLP 6000 pressure monitor; 15) Line pressure separator; 16) 1/4 inch Y connectors. PACKAGING REQUIREMENTS: All packs (Items #1-16) shall be packaged for a sterile environment in surgery as these packs are used in conjunction with the heart bypass machine (Sarns 8000) for coronary artery bypass graft cases. If your business is capable of providing the above to the VA Ann Arbor Healthcare System, Ann Arbor, MI, please respond to this Sources Sought Notice no later than 3:30 PM EST, Friday, March 17, 2006.
