- The VA Medical Center, 200 Veterans Avenue, Beckley, West Virginia, is seeking potential sources to provide HOMEMAKER/HOME HEALTH AIDE SERVICES to eligible veterans living in the Beckley VA Medical Center primary service area. The Beckley VA Medical Center participates in a veteran?s health administration program to provide homemaker/home health aid (H/HHA) services to eligible veterans, who in the absence of H/HHA support services would require nursing home placement. H/HHA services will be furnished to veterans in the community for whom such care is specifically authorized by the Beckley VA Medical Center. The services may include home health aides, personal care aides, and homemakers. Beneficiaries of this contract shall be any eligible veteran living in the Beckley VA Medical Center?s primary service areas as follows (counties): Clay, Fayette, Greenbrier, McDowell, Mercer, Monroe, Nicholas, Pocahontas, Raleigh, Summers, and Wyoming counties in West Virginia and Tazewell county, Virginia. Services are needed for a period of twelve months from the date of award with four one-year renewal options. Contractor shall provide all personnel, equipment, tools, materials, transportation, management supervision, and other items and services necessary to perform all tasks and functions in accordance with all terms, conditions, provisions, specifications, and schedules of the solicitation, pending the availability of funds. The following Current Procedural Terminology (CPT) codes are provided for procedures allowed under this contract: 99510, 99509 and 99342. The applicable NAICS Code is 621610 with a size standard of $11.5 Million. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company/provider; business size; socio-economic category, if any; if services are on a Federal Supply Schedule; firm?s experience, and capability of expeditiously beginning services on April 1, 2006, and any other pertinent information which demonstrates firm?s ability to meet the above requirement.
