- NAICS Code: 334210 The Environmental Protection Agency (EPA) intends to award a sole source, Fixed-Prices contract to Sprint -Public Sector 330 South Valley View Blvd Las Vegas, NV 89107, for telecommunications services at its Emergency Response Team-West Headquarters (ERT West) in accordance with FAR Part 6.302-1. The services are required for a period of 12 months at ERT West, located at 4220 South Maryland Pkwy, Bldg D, Suite 800 Las Vegas, NV 89119. The start date for the services is April 1, 2006. The services include: 1. 12 Month Contract Term Agreement 2. ISDN Voice Data Channel 3. ISDN PRI Channel 4. ISDN PRI B Channel, Quantity 23 5. Additional 100 Numbers 6. DID Additional 50 Numbers 7. Local Directory Assistance 8. Interstate Access Charge 9. Federal Service Charge 10. In-State Directory Charge 11. National Directory Assistance Charge 12. NV Universal Service Fund Charge 13. Customer Service Phone Numbers (702-733-0040, 702-733-0512, 702-733-0555, 702-733-0642) 14. Wireless Portability Surcharge Any interested party that can provide ALL of the services above, can submit a quote by 12 Noon March 8, 2006 Eastern Standard Time. Quotes will be evaluated on a lowest price, technically acceptable basis. The evaluation criteria for this procurement are Technical Capability, Price, and Past Performance. Evaluation criteria will be rated as pass/fail. The list above may not be all inclusive of fees.
