GLENN, JR./ (478) 926-6358 or fax at (478) 926-7549 e-mail at John.Glenn@robins.af.mil Sole Source Acquisition to: LK METROLOGY, 12701 GRAND RIVER, BRIGHTON MI 48116 DESCRIPTION OF REQUIREMENT: The Contractor shall provide all the labor, material, supplies, and transportation necessary to perform annual maintenance for four (4) Coordinate Measuring Machines located at Robins AFB GA in strict accordance with the incorporated Statement of Work. PERIOD OF PERFORMANCE: Basic year, with four (4) option years FOB: Destination THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6 AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. The following clauses are applicable. 52.252-2 -- Clauses Incorporated by Reference (Feb 1998) This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Applicable Law for Breach of Contract Claim. 11. 52.237-1 -- Site Visit 12. 52.237-2 -- Protection of Government Buildings, Equipment, and Vegetation 13. 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items Statement of Work ANNUAL SERVICE CONTRACT for Coordinate Measuring Machines (CMM) PHYSICAL SCIENCE LABORATORY ROBINS AFB, GA 31098 A. SCOPE: The Materials Section within the Physical Science Laboratory requires an annual Service Contract for four (4) each Coordinate Measuring Machines (hereafter called CMM). The CMM shall be serviced in accordance with established LK Metrology Systems Corporation policies and procedures B. REQUIREMENTS: 1. The Contractor shall provide all labor and replacement parts for all mechanical and electrical machine and controller components, as required, for on-site visit for problem resolution in the event that any of the four (4) CMM experiences operational failure. In addition, the Contractor shall provide two (2) preventative maintenance visits semi-annually to ensure that the CMM are fully capable of performing all operational tasks as originally designed. a. In the event that the CMM experiences operational failure, the Contractor?s service representative shall perform diagnostic tests to determine the cause of failure and what repairs need to be made. b. Once determined, the operational status or repair/service requirements of the CMM shall be discussed with the Government POC in Bldg 323, RAFB, GA. c. Only LK approved parts or components or assemblies shall be used in the service and/or repair of the CMM. d. Two (2) Preventative Maintenance visits annually shall be scheduled by the Contractor to service the CMM. The dates of the PM visits shall be agreeable to the Government POC. 2. The Contractor shall provide all travel, per diem, and labor related to the service and/or repair of the CMM. a. All work shall be accomplished in Bldg 323, RAFB, GA, during normal business hours. There is no government requirement to accomplish work at any time other than normal business hours (i.e. No work during weekends, holidays, or after normal business hours). b. Only trained and qualified LK Corporation service engineers and/or technicians shall perform service and/or repair on the CMM. C. EQUIPMENT: 1. The equipment to be serviced is as follows: (a.) Peripheral equipment such as air compressors, power conditioners, and air dryers shall not be serviced nor repaired. D. TESTING: 1. After servicing and/or repair of the CMM, the Contractor shall conduct a performance test to demonstrate the performance capability. 2. This test shall be observed and approved by the Government POC at Bldg 323, RAFB, GA. E. QUALITY ASSURANCE AND DOCUMENTATION: 1. When the service or repair is completed, the contractor shall provide a clear and concise service report to the Government POC at Bldg 323, RAFB, GA. 2. The service or repair shall be covered by the contractor?s standard warranty for at least a period of ninety (90) days.
