Description

The Carl T Hayden VA Medical Center is seeking Contractors to operate and provide Community Based Outpatient Clinic (CBOC) services for the Department of Veterans Affairs in Payson, Arizona and in Buckeye, Arizona. The facility shall be physically located within the city limits of Payson, Arizona (Gila County) and Buckeye, Arizona (Maricopa County). Contract personnel shall provide primary care services and mental health services for eligible enrolled VA beneficiaries. Estimated number of VA beneficiaries in Payson is 600 and the estimated number of VA beneficiaries in Buckeye is 300. Historically, the CBOC contract has been priced on a per member/per month/ capitated basis. Contract period is date of award for one base year with four option year renewals. The NAICS is 621498; size standard is $8.5M.
