Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Written quotes are being requested. Quotes may be submitted on the Standard Form 18 (Request for Quotation). STATEMENT OF WORK TITLE: U. S. Environmental Protection Agency (EPA) Diver Training Program, May 8-12, 2006 BACKGROUND: Participation in diving activities under EPA auspices requires certification through the EPA Diver Training Program. Requirements for certification vary according to the certification category and usually involve a combination of classroom training and open water experience. The EPA Diver Training Center, located at the EPA’s Gulf Ecology Division, Gulf Breeze, FL, offers a variety of dive training courses, which are open to EPA employees, as well as personnel of other governmental agencies. Courses offered by the GED Diving Center include: • Scientific/Working Diver • Trainee Diver • Dive master • Nitrox Diver • Diving Accident Management Divers participating in the U.S. EPA Diver Training Program must have completed basic SCUBA training and logged a minimum of 25 dives. The extensive one-week Diver Training Program is designed to train EPA employees in Diving Accident Management, Scientific/Working diving, Nitrox diving, and contaminated water diving. The program exposes the divers to a variety of equipment, tasks, and situations typically encountered by scientific divers. DESCRIPTION OF WORK: The contractor shall conduct a Diver Training Program Monday through Friday beginning at 8:00 a.m., May 8, 2006 and ending at 4:00 p.m., May 12, 2006, containing current information on techniques for compressed gas underwater operations in both "clean" and "contaminated" water. The contractor shall provide a course appropriate for divers who are currently certified with basic scuba certification and higher levels. The course shall be conducted by instructors who have demonstrated expertise in training divers in diving accident management, scientific diving, nitrox diving, introduction to surface supplied air, polluted water diving awareness, Superlite-27, EXO-26 mask, AGA masks and field repair of AGA masks and drysuits. The contractor shall provide training for a maximum of 24 working divers and 6 dive master candidates. The training course shall comply with EPA Diving Safety Manual, Version 1. The content of the Training Program shall be consistent with NOAA Diving Manual (Fourth Edition). The contractors shall provide a detailed course outline to be evaluated with their proposal. (Contact Contracting Officer listed below for copy of Attachment 1 - Sample Outline). The contractor shall arrange a one-day rental of a swimming pool for the swimming test within 20 miles of GED (e.g., Langley Street YMCA). Pool reservations, diver training projects, dock area, and classroom must set up by May 5, 2006. The contractor shall provide instructions in the following areas: 1. Scientific Diving Exemptions and Regulations; 2. Dive master Duties; 3. Working diver responsibilities; 4. Physics, Physiology, Pathophysiology and Medical Aspects of Diving; 5. Field Management of Diving Accidents; 6. Awareness for Diving in Contaminated Water including a. Contaminated Water Diving Dress and b. Decontamination Procedures for Chemical, Fuel, Biological, or Sludge (provide pressure washer); 7. Oxygen-Enriched Air Diving Procedures including Physics, Physiology Decompression Problems a. Mixing, Oxygen and Oxygen Safety and b. NITROX Training and Certification; 8. Use of Dry Suits including a. Emergency Safety Techniques and b. Decontamination and Storage; 9. Proper Use of the Following Equipment: a. Surface Supplied Diving with NITROX, b. U/W Communications (wireless and hard wire), c. Search and Recovery, d. U/W Tools (hand tools, lift bags, metal detector, e. Pinger Locator (Sonar), f. Diving Helmets (superlite- 27), and g. Full Face Masks (A.G.A., EXO-26); 10. Training and Qualifications in the following: a. Dive master training including field service of regulators (2nd stage), AGA masks and dry suits, b. Scientific Diver / Dry Suit Training / Search and Recovery, c. Trainee Diver, d. Diving Accident Management including in-water rescue, e. Contaminated Water Diving, f. Oxygen Therapy & Administration Certification for 45 g. NITROX, and h. Hazardous Materials Training for Breathing Gases. The contractor shall assemble training projects 1 week before training (Contact Contracting Officer listed below for copy of Attachment 2). The contractor shall be responsible for the health and safety of the individuals participating in the Diver Training Program. The contractor shall examine the government diving equipment(GDE), complete standard annual maintenance procedures and make necessary repairs. The contractor shall provide a written report describing equipment maintenance and repair to the government representative by May 5, 2005. A written certification that the GDE, contractor and government furnished equipment complies with OSHA safety standards shall be included in this report. The following tasks shall be completed to meet this requirement: 1. Pressure test three (3) surface supply hoses. Conduct diver umbilical maintenance on two (2) 150 ft. hoses and one (1) 50 ft. hose; 2. Visually inspect fifty (50) air cylinders (VIP) and fill. Segregate and report defective cylinders; 3. Inspect twelve (12) AGA masks. Test all AGA's with mfg. test kit and conduct annual maintenance; 4. Inspect three (3) O2 regulators and provide maintenance, if necessary; 5. Fill six (6) O2 cylinders, and hydrostatically test four (6) of these O2 cylinders; 6. Conduct maintenance on Superlite 27. Inspect side block manifolds and provide maintenance; 7. Perform maintenance on two (2) DCS-3 (DSI Dive Control System); 8. Provide 4-pin adapter for communications on EXO-26 mask and annual service on mask; 9. Repair up to four (4) drysuits if damaged during the training exercise (e.g., torn cuffs, neck seals, broken valves, or replace zipper(s); The contractor shall submit a time-line for completing the various tasks (e.g., inspecting and repairing GFE, preparing diver training projects, preparing dock area and classroom, etc.). The contractor shall provide a summary of experience in Diving Accident Management, Scientific Diving, Nitrox Diving, Introduction to Surface Supplied Air, Polluted Water Diving Awareness, Proper Use of Superlite-27, Proper use of UEXO-26 Masks, Proper Use and Field Repair of AGA Masks and Drysuits. The summary shall include an outline of the instructor experience, current training courses and years of experience. A resume shall also be included. The contractor shall provide a list, including names and telephone numbers of prior clients to whom similar courses have been provided. The contractor shall disclose any conflict or potential conflict of interest that he/she may have in connection with the Federal government. The conflict of interest form must be signed and included with the proposal. GOVERNMENT FURNISHED ITEMS The US EPA will provide the following facilities and equipment for use in this training: 1. 12' screen and 20" TV with VHS VCR 2. Infocus projector for Power Point presentations 3. Slide and overhead projector 4. Docks 5. Boats (operated by EPA personnel), 6. 25 diving cylinders 7. 12 AGA masks 8. 1 diver recall, 9. 1 O2 kit 10. 12 dry suits 11. 1 Superlite 27 helmet 12. 1 surface supplied air system and regulators. The contractor shall provide all other equipment required for this Training Program. CONTRACTOR FURNISHED ITEMS The contractor shall provide approved study materials including thirty (30) copies of each of the following: 1. Steve Barskey’s Diving in High Risk Environments; 2. Florida’s Underwater Council’s Diving Accident Management or equivalent manual; 3. Folder with pad and pen; 4. Diving Regulations (original copy can be obtained from the government representative); 5. Three (3) worksheets for homework consisting of 20 questions on diving physics, physiology, and decompression problems; 6. NITROX Manual; 7. NITROX Certification cards 8. Diving Accident Management Certificates (example provided by government representative). The contractor shall provide the following supplies and equipment: 1. Approx. 200 cylinders of breathing gases (50 cylinders each day for 4 days); 2. Diver Rescue Oxygen Kits (2); 3. Oxygen analyzers (2); 4. AGA Masks (12); 5. Superlite-17 or equivalent diving helmet; 6. Dry Suits (12); 7. Surface Supplied Air Systems (2); 8. 1" box end wrenches (4); 9. Equipment to complete approved underwater projects (Contact Contracting Officer listed below for copy of Attachment 2); 10. First Aid Equipment for first responder to dive accidents (O2, backboard, diver first aid kit); 11. Basic Dive Equipment (regulator, vest, wt. belt, mask, fins, etc.) for 1 contract diver who may be needed as a standby diver or to deploy and retrieve projects; 12. Safety Equipment (2 dive flags, 6 buoys, 10’ line for each buoy); 13. Field Repair Kits (2) for AGA & Dry Suits; 14. Gases sufficient to complete the Training Program including: a. Approx. 16,000 cubic ft. of breathing gas (each day during the training at least 50 cylinders (80 Cu. Ft.) of breathing gas shall be delivered), and b. 32% Oxygen Enriched Air for NITROX certification of 24 candidates; and 15. Provide supplies necessary for the activities within this statement of work such as o- rings, adapters, spare hoses, etc 16. To ensure safety and identification through out the course, the contractor shall provide Identification gear for 10 Instructors, 6 Divemasters, and 24 Divers. Navy blue t shirts with large yellow lettering on the back. These will be returned to contractor after the training. The following provisions apply: 52.212-1 - Instruction to Offerors – Commercial – 52.212-2 - Evaluation of Commercial items The evaluation for this procurement will be based on technical, price and past performance. 52.212-3 - Offeror Representations and Certifications – Commercial Items (offer must include a completed copy of this provision.)
