Description
This is a combined synopsis solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation HHSP284-R-2006-04 is issued as a Request for Proposal (RFP). The solicitation incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-07. This acquisition is a total Small Business Set-aside and the small business size standard is 500 employees. The North American Industry Classification Code is 326140 Polystyrene Foam Packaging Manufacturing. A proposal is requested for three (3) line items to be delivered on a monthly basis: LINE ITEM 1: Container, Insulated Shipping, Styrofoam with Lid Outside dimensions:Minimum Length:9 Maximum Length: 10? Minimum Width: 11? Maximum Width: 12? Minimum Height: 12? Maximum Height: 14? Minimum wall thickness: 1.5 inch Quantity per month: 288 each Quantity per year: 3,456 each; LINE ITEM 2: Container, Insulated Shipping, Styrofoam with Lid Outside Dimensions:Minimum Length: 15? Maximum Length: 17? Minimum Width: 14 Maximum Width: 16? Minimum Height: 12? Maximum Height: 14? Minimum wall thickness: 1.5 inch Quantity per month: 144 each; Quantity per year: 1,728 each; LINE ITEM 3: Container, Insulated Shipping, Styrofoam with Lid Outside dimensions: Minimum Length: 22? Maximum Length: 24? Minimum Width: 15? Maximum Width: 16? Minimum Height: 15? Maximum Height: 16? Minimum wall thickness: 1.5 inch Quantity per month: 60 each Quantity per year: 720 each. Inspection will be made at the delivery point and will consist of random sampling of the product for conformance with specifications and quantity count. It is anticipated that the Government will make a single award on an ?all or none? basis. Offeror must specify how materials will be packaged for delivery. Delivery terms shall be FOB - Destination to HHS, PSC, Supply Service Center, Bldg. # 5, Perry Point, MD 21902. All transportation shipping costs are to be included in the proposal. All deliveries shall be made monthly on regular workdays (Monday through Friday) during regular business hours (7:30 a.m. to 3:30 p.m. Eastern Standard Time), excluding official Federal Government holidays. Monthly quantities may be adjusted to provide for full pallet/skid quantities. Monthly delivery schedule will be established at time of award. The provision at 52.212-1, Instructions to Offerors ? Commercial (Jan 2006), applies to this acquisition. Offers will be evaluated on the basis of technical capability to meet the Governments requirement and price. Offerors shall submit a fully executed copy of provision 52.212-3, Offeror Representations and Certifications Commercial Items (Mar 2005) with their offer.
