Description
t. Chief Nurse: 1. The contract Chief Nurse shall be required to assist the staff physician with treatment; assess patient health problems and needs; develop and implement nursing care plans; maintain medical records; and assist patients in complying with a prescribed medical regimen. The chief nurse shall oversee the staff nurse's performance and assist with any and all medical treatments necessary to operate the specified facility. This chief nurse shall have knowledge of special drugs and the ability to provide pulmonary ventilation. 2. Education. The Chief Nurse shall be: (a) A graduate of a baccalaureate degree nursing program; (b) Certified occupational health nurse (COHN) by the American Board of Occupational Health Nurses (ABOHN) (1) Licensed to practice in the Washington, DC metropolitan area (DC, MD, and VA) or in the areas the services will be performed; (2) Certified as occupational hearing conservationist by the Council for Accreditation in Occupational Hearing Conservation (CAOHC); and (3) Certified to administer the American Heart Association modular cardiopulmonary resuscitation (CPR) 3. Experience. The Chief Nurse shall have: (a) Experience in occupational health nursing with supervisory experience; (b) Experience in an industrial medical program, or in ambulatory or emergency care; (c) Experience in counseling troubled employees; and (d) Experience in conducting special medical programs or health education. u. Staff Nurse: 1. The contract Staff Nurse shall be required to assist the chief nurse with treatment such as, but not limited to, administering immunizations, inoculations, allergy treatments, and medications at the health and wellness centers. The staff nurse shall assist the chief nurse in developing, evaluating, and revising nursing plans. 2. Education. The Staff Nurse shall be: a) A graduate of a baccalaureate degree nursing program b) A Certified Occupational Health Nurse (COHN) by the American Board of Occupational Health Nurse (ABOHN) c) Licensed to practice in the Washington, DC metropolitan area (DC, MD, and VA) or in the areas the services will be performed. 1) Certified as occupational hearing conservationist by Council for Accreditation in Occupational Hearing Conservation (CAOHC) 2) Certified to administer the American Heart Association modular cardiopulmonary resuscitation (CPR) 3. Experience. The Staff Nurse shall have: Professional nursing experience in an industrial medical program, or in ambulatory or emergency care; b) Experience in counseling troubled employees; and c) Experience in conducting special medical programs or health education. v. Medical Secretary: 1. The contract Medical Secretary shall be required to compile, verify, prepare, file, and maintain medical records of employees and compile statistics for use in reports and surveys. He/she shall review records to insure presence of required reports and physicians' signatures, and route incomplete records for completion or prepare a report of incomplete records to notify appropriate personnel. The medical secretary shall compile daily and periodic statistical data, such as, but not limited to, admissions, deaths, and types of treatment rendered. In addition, any and all clerical duties needed to fulfill the agency's medical requirement shall be accomplished by the secretary. 2. Education: The Medical Secretary shall have a high school diploma or equivalent. 3. Experience. The Medical Secretary shall have: (a) Experience as a secretary/typist. (b) Certified to administer the American Heart Association modular cardiopulmonary resuscitation (CPR) w. Key Personnel: Key personnel are understood to be those individuals for whom resumes were submitted as part of the proposal to this solicitation. Any substitution in the key personnel, temporary or permanent shall be replaced by those persons whose resumes and credentials were submitted with this proposal as back-up personnel and who are necessary to fill the requirements of the contract. No substitutions shall be made except in accordance with the back-up personnel clause below. x. Back-up Personnel: In the absence of key personnel listed, the COTR shall receive prior written notification when key personnel are absent. Prior to utilizing other personnel specified in the initial proposal, the CO and the COTR shall be notified no later than 30 days in advance and shall submit justification, resume, license, and certifications to the Contracting Officer for approval. Prior to removing, replacing, or diverting the key personnel, the Contractor shall promptly notify the Contracting Officer. All proposed substitutions shall be submitted to the Contracting Officer, in writing, for approval at least 30 calendar days in advance of the proposed effective date. All requests for substitutions must have a complete resume of the proposed substitute, and any other information requested by the Contracting officer. All substitutions must have equal or better qualifications than those of the person they are replacing. (If the substitution is necessitated by sudden illness, death, or termination of employment, notice shall be made within five calendar days of the action.) y. Permits, Licenses, and Insurance: During performance of any contract awarded in response to this Solicitation, the Contractor shall maintain liability insurance coverage, from a carrier acceptable to the Department of Veterans Affairs, for all personal injuries, property damage and/or wrongful death arising from performance of the Contract. Prior to beginning performance, the Contractor shall provide the CO with certificates evidencing said insurance coverage. During performance of any contract awarded in response to this Solicitation, the Contractor shall indemnify and hold harmless the federal government all claims for personal injury property damage and/or wrongful death arising from performance of the contract. Fitness Center Scope of Work The contractor shall provide the necessary personnel, management, and equipment required to operate the Department of Veterans Affairs physical fitness facility located at 810 Vermont Ave., NW, Washington, DC. These services will be available to approximately 2,900 VA employees and authorized non-VA personnel utilizing the facility. The Contractor will be required to maintain accurate and complete fitness records, provide supplies, coordinate VA equipment purchases, and oversee special fitness programs sponsored by VA (e.g., National Fitness Month, Race for the Cure, etc.). The Fitness Center shall be open Monday through Thursday from 6 a.m. to 7 p.m., and Friday, 6 a.m. to 6 p.m., except Federal government holidays, building closures, or early dismissals. The Fitness Center shall not be closed at lunchtime unless specific approval is received from the Contracting Officer's Technical Representative (COTR). In the event VA Central Office must close for the entire day or delay opening due to weather conditions, an announcement will be broadcast on local TV and radio stations. Personnel 1. General. In their performance of the contract, the Contractor shall utilize the personnel set forth in the Contractor's proposal, including proposed back-up personnel. Resumes shall be submitted for each proposed personnel and their back-ups. Prior to utilizing other personnel specified in your initial proposal, the Contractor shall notify the Contracting Officer (CO) and the COTR 15 calendar days in advance and shall submit a justification and resume in sufficient detail to permit evaluation on the impact of contractor performance. Pending Government review and approval, no substitution shall be made by the Contractor without the approval of the CO. In the event contractor personnel performance is deemed unsatisfactory, the CO reserves the right to request and receive fully qualified replacement personnel at no additional cost to the Government. 2. Staffing Requirements. The Contractor, at a minimum, shall provide one (1) full time Physical Fitness Program Coordinator and one (1) full time Physical Fitness Assistant (e.g., cooperative education college student). In addition, the Contractor may, on a periodic basis, be required to provide additional labor hours (CLIN's 003 and 004) for additional personnel under this contract. In addition, the Contractor shall ensure that at least one contractor personnel is present at each Fitness Center at all times during operation hours. The Contractor shall provide, at a minimum, fully qualified personnel for the categories and quantities listed below to perform required services, and shall be responsible for certifying that they meet the personnel qualifications listed in this solicitation. Each proposed candidate must meet, at a minimum, the requirements for the positions specified. The Contractor agrees to provide backup personnel for positions listed above when contract personnel are absent due to leave or sickness (at no additional cost to the Government). If contract personnel call in sick, the Contractor shall guarantee backup personnel within four hours (the Contractor shall not charge the Government for labor hours that contractor personnel are absent). For any other leave taken by proposed full or part time personnel, the Contractor shall pre-arrange necessary backup personnel that will not result in a break in required services. In addition, the Contractor shall notify the COTR by telephone when personnel are absent. All backup personnel shall meet applicable personnel qualifications listed in Section C. Absence of Physical Fitness Program Coordinator: To the extent possible, the COTR will receive prior notification (by telephone) when the Physical Fitness Program Coordinator is to be absent. The proposed backup Physical Fitness Program Coordinator shall be made available within four (4) hours of primary Coordinator's absence. Until the backup personnel arrives, the Physical Fitness Assistant will ensure that the Fitness Center is opened on time. Personnel Qualifications 1. Physical Fitness Program Coordinator. Since funding for the VA Fitness Center is completely dependent on membership fees, the Physical Fitness Program Coordinator shall use a variety of marketing strategies to promote the use of the Fitness Center by VA employees. a. Education: (1) A graduate of an accredited college with a Bachelor of Science degree in Physical Education, Kinesiology, Exercise Physiology, or related degree; and Certified instructor with the American Heart Association Modular System of CPR training. b. Experience: (1) Two years professional experience that includes: experience in physical fitness facility, providing health education programs, counseling in physical fitness and nutrition, knowledge and use of exercise equipment (including ability to make minor repairs and adjustments to exercise equipment); (2) One year professional experience in managing a physical fitness facility; (3) Six months experience in coordinating and conducting aerobic exercises (e.g., Jazzercise, etc.); Three months experience in general fitness assessment, including: sub-maximal aerobic capacity, flexibility, muscular endurance, skin fold thickness and body fat, body circumference, height, weight, resting and dynamic heart rate, and resting blood pressure; and (5) Three months experience in exercise prescription and computerized record keeping. 2. Physical Fitness Assistant a. Education: (1) One year or more college education pursuing a Bachelor of Science degree in Physical Education, Kinesiology, Exercise Physiology, or related degree; and (2) CPR certified. b. Experience: (1) Three months experience working in a Fitness Center; (2) Three months experience in use of aerobic exercise equipment; (3) Three months experience in improving cardio-respiratory endurance, muscular endurance, and joint flexibility; (4) Demonstrate proficiency with a personal computer and typing skills; and (5) Demonstrate ability to administer emergency and first aid procedures until the arrival medical assistance. 3. Additional Qualifications for all Fitness Center Contractor Personnel. Contractor personnel shall be in excellent physical condition. Contract personnel may be encouraged by the contractor to utilize the fitness center equipment during non-peak use hours. On a quarterly basis, the contractor shall require contract personnel to pass a standard fitness test (e.g., endurance, strength, agility, and flexibility), and certify to the COTR that the contract personnel passed the test. This shall ensure that contractor personnel are knowledgeable in the use of equipment located at the Fitness Center, and to instruct VA personnel on many of the exercises. In addition, these standards of physical conditioning will ensure contractor staff is able to participate in most walking, running, biking, etc. for the various fitness programs VA may be involved in. Finally, Contractor personnel should be familiar with jogging and bike trails in Washington, DC. Contractor Furnished Property. The Contractor shall be responsible for purchasing and providing identifying uniforms (e.g., collared knit shirts with company color, name/logo, and gym pants/shorts) that will distinguish them as fitness center personnel versus Government employees utilizing the center. In addition, the Contractor shall be responsible for any miscellaneous supplies required in the Fitness Center (e.g., paper and toner for personal computer printer, general office supplies, cleaning supplies, water, etc.). Maintenance of Records. The Contractor shall maintain secure, accurate, and organized records (this includes a manual describing the filing and inventory listing all individual fitness files) of all Government employee files during the performance of this contract. The record system shall be kept up-to-date and shall be made available to the COTR, upon his/her request. Government Furnished Property. The following Government Furnished Property (GFP) will be provided for the Contractors use under this contract. The Contractor shall be responsible for the cost of maintaining and repairing the following Government Furnished Property, including any additional equipment purchased under this contract. The Contractor shall operate the physical fitness facility and administer a health enhancement program for approximately 300-350 employees and authorized non-VA persons that are currently utilizing the facility per month with an average of 80-100 Individuals utilizing the facility daily. Ensuring that the Fitness Center opens promptly at 6 a.m., Monday through Friday, except Federal government holidays or building closures. Providing guidance and assistance to program participants, medical clearance and monitoring. In addition, the Contractor shall provide observation and supervision of participants while exercising to avoid excess stress, and nutritional counseling. Ensuring that the Fitness Center is being monitored during regular hours by Contractor Personnel. The Contractor shall maintain open avenues of communication with the VA Central Office Health and Wellness Centers in assessing its individual employee's physical condition and providing results of individual stress tests. Ensure that each individual utilizing the Fitness Center has filled out a form detailing medical history, self assessment of physical capabilities, and has received a stress test administered by Contractor personnel. Advertise, coordinate, facilitate, and instruct aerobic exercise classes on a daily basis: this includes a minimum of one and a maximum of three daily sessions, 30 minutes in length, of aerobic classes (e.g., step, jazzercise, etc.) led by contractor's personnel (depending on Government employee interest). As needed, advertise, coordinate, and, participate in any special programs that VA may be involved in to promote physical fitness (e.g., National Fitness Month, Presidential Physical Fitness, Race for the Cure, walk- a-thon, etc.). Monitor individuals utilizing the Fitness Center and, on a regular basis, check with those users who have unique physical conditions identified by the VA Central Office's Health and Wellness Center or prescribed by private care givers, and develop an individual exercise program. On a periodic basis, check exercise and electronic equipment to ensure they are in proper working condition, gauges and indicators calibrated, and maintained regularly. Provide on a bi-weekly basis, updates on a bulletin board: local athletic opportunities (e.g., walking, running, biking, swimming, etc.). Act as a facilitator and central place for exchange of information between VA employees for softball leagues, and other fitness activities in Washington, DC. As needed, maintain a clean and sanitary environment for the operation of the Fitness Center, including daily cleaning of contractor space (mopped, equipment cleaned with disinfectant, dust free), neat and orderly appearance. In addition, inspect the locker rooms and showers for cleanliness and report any issues to the COTR. Maintain accurate records that will include physical fitness goals of individual users. Administration of first aid and cardiopulmonary resuscitation and have prominently displayed established methods for obtaining medical assistance and transferring a person with a serious illness or injury to a hospital. Maintenance of a sign-in system for all Fitness Center users (including authorized non-VA personnel utilizing the facility), and on a quarterly basis the tabulation and preparation of statistical summaries of Fitness Center use. Note: Ensure that regular and backup personnel are trained in all areas of above listed operations in the Fitness Center so that the program is adequately managed. In addition, these personnel should be regularly trained (e.g., current certifications) in: first aid and cardiopulmonary resuscitation techniques. Finally, they must have knowledge of how to obtain medical assistance and transferring a person with a serious illness or injury to a hospital. Reporting Requirements: 1. Monthly Work Achievement Report: The Contractor shall submit Monthly Work Achievement Reports to the COTR containing information as set forth below on the 10th day after the end of the reporting period. As a minimum, it will contain: a. Total visits to the Fitness Center. b. Average daily visits to the Fitness Center. c. Fitness orientation/assessments completed. d. Fitness Reassessments completed. e. Individual employees using the Fitness Center that month. f. Regular Users that month. g. Average number of employees at peak period. 2. Technical Reports: The Contractor may prepare technical reports on the work performed under its contract when considered appropriate by the Contractor, due to special findings or upon the completion of a phase of work. Technical reports relative to the work performed under this contract may be published by the Contractor in technical journals or presented at technical meetings, provided that information copies are submitted to the COTR 30 days in advance of the publication along with the name of the technical journal or group to which the information is presented. Final Report: The Contractor will submit a final report to the COTR which documents and summarizes the results of the entire contract work, including recommendations and conclusions based on the experience and results obtained. A draft report will be submitted to the COTR two months in advance of contract expiration. This will be reviewed by the COTR and upon his/her approval; the Contractor will finalize the report and submit it 30 days in advance of contract expiration. Slgaskins 1/20/06
