Description
The Bureau of Prisons is seeking a Request for Quote from sources having the ability to provide OUT-PATIENT assessment and counseling and Mental Health Services in the geographically defined district of Youngstown, Ohio for the drug abuse treatment services for male and female Federal offenders (inmates) held under the authority of various United States Statutes. The geographically-defined area follows: Within the confines of Midlothian Boulevard; Powers Way; Powers Way; Route 62; Poland Avenue; Route 62; Albert Street; Redondo Boulevard; Route 193; Route 680; Glenwood Avenue, and local public transportation route. Performance period under this contract will be for a one year base period to begin on October 1, 2006 with four (4) one year option periods, with service ending (if all option years are renewed) on September 30, 2011. The services required for this contract include: Base Period, Option Year One, Option Year Two, Option Year Three, and Option Year Four, respectively: Base Period - Intake Assessment (1 per inmate for 29 inmates) 29; Individual Counseling (16 units per inmate for 29 inmates) 464; Group Counseling (50 units per inmate for 29 inmates) 1450; Mental Health Intake Assessment and Report (1 per inmate for 5 inmates) 5; Medication Monitoring (2 units per inmate for 5 inmates) 10. Option Year One - Intake Assessment (1 per inmate for 30 inmates) 30; Individual Counseling (16 units per inmate for 30 inmates) 480; Group Counseling (50 units per inmate for 30 inmates) 1,500; Mental Health Intake Assessment and Report (1 per inmate for 5 inmates) 5; Medication Monitoring (2 units per inmate for 5 inmates) 10; Option Year Two - Intake Assessment (1 per inmate for 31 inmates) 31; Individual Counseling (16 units per inmate for 31 inmates) 496; Group Counseling (50 units per inmate for 31 inmates) 1,550; Mental Health Intake Assessment and Report (1 per inmate for 5 inmates) 5; Medication Monitoring (2 units per inmate for 5 inmates) 10; Option Year Three Intake Assessment (1 per inmate for 32 inmates) 32; Individual Counseling (16 units per inmate for 32 inmates) 512 ; Group Counseling (50 units per inmate for 32 inmates) 1,600; Mental Health Intake Assessment and Report (1 per inmate for 5 inmates) 5; Medication Monitoring (2 units per inmate for 5 inmates) 10. Option Year Four - Intake Assessment (1 per inmate for 33 inmates) 33; Individual Counseling (16 units per inmate for 33 inmates) 528; Group Counseling (50 units per inmate for 33 inmates) 1,650; Mental Intake Assessment and Report (1 per inmate for 5 inmates) 5; Medication Monitoring (2 units per inmate for 5 inmates) 10. The services described shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work, dated December 2005. The solicitation will be available on or after March 8, 2006, and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http://www.fedbizopps.gov/.
