- These specifications set forth the requirements for electronic pre-press, proofing, printing, binding, packing, labeling and delivery of 1,600,000 copies of a five-part form, top stub, printing face only and face and back in black ink. Forms deliver to 2 locations. 533,334 forms MUST deliver by 3/1/06. An additional 533,334 form MUST deliver by 3/10/06. The balance of 533,332 forms MUST deliver by 3/20/06. Overall size is 8-1/2 x 12" including the 1/2" top stub. The bottom part of part 3 is an 8-1/2 x 5" envelope. Parts 4 and 5 join to form an 8-1/2 x 11" envelope. Adequate gripper margins.
