- In section (xi) the offer due date is corrected to read 13 February 2006. (i) THIS IS A COMBINED SYNOPSIS/SOLICITATION for Commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation number N68335-06-T-0004 is issued as a competitive request for proposal (RFP). This is set aside for small business. (iii) The solicitation and incorporated provisions and clauses are in effect through Federal Acquisition Circular 97-26. (iv) Contract Line Item Numbers (CLINS) are as follows: CLIN 0001 AMAD Blank Off Plate and Kit (see attached SOW and drawings), Qty 50-75. CLIN 0001 consists of the follows: 4 Oil Transfer Tubes for each Blank Off Plate; 1 Pipe Plug for each Blank Off Plate; 1 Oil Flow Orifice Plug for each Blank Off Plate; and 1 Case for each Blank Off Plate. Provide unit prices for a quantity of 50 and for a quantity of 75 Blank Off Plate and Kit. (v) All items shall be preserved, packaged, packed, and marked in accordance with best commercial practices. Provide delivery time with the proposal. FOB Destination. Ship all items to: Naval Air Warfare Center AD, Support Equipment Staging Area, Bldg 195 (Hangar 6), Hwy 547, Lakehurst, NJ 08733, M/F: Jeff Clark, Code 4861, (732) 323-1944. (vi) FAR 52.212-1 Instructions to Offerors ? Commercial Items, is incorporated by reference. (vii) FAR 52.212-2, Evaluation ? Commercial Items is incorporated by reference. Addendum 52.212-2 Para (a): Award will be made to the lowest priced, technically acceptable offeror. (viii) Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items and DFARS 252.212-7000 Offeror Representations and Certifications - Commercial Items (NOV 1995) with its offer. (ix) FAR 52.212-4, Contract Terms and Conditions - Commercial Items is incorporated by reference. The resulting contract will be firm fixed price (FFP).
