- Description
- Provide back-up Speech Pathology Services at the VA Salt Lake City Health Care System as per the terms and conditions of the contract. Contractor will be required to review medical charts, request medical records, interview Veterans, Counsel Veterans, attend team treatment/planning meetings, provide written documentation of evaluation and treatment provided to patients, provfide supervision of students from affiliated universities,and administer speech pathology tests. The contractor is required to have the following qualifications with documentation: have a Masters degree in Speech Pathology, CCC-Sp American Speech Language & Hearing Association, have a state license in speech pathology, and pass the credentialing and privileging process of the VA Salt Lake City Health Care System. The contract will run from the date of award through February 28, 2007 with four one-year options to extend the term of the contract. The contract will utilize simplified acquisision procedures in acordance with Commercial items Part 12 and in accordance with Part 15, Contracting by Negotiation. The issue date is O/A February 10, 2006 - BOD O/A 02-27-2006 The contract will be an IDIQ contract.
