See above Description for CLINS 0001-0001AN Section C - Descriptions and Specifications STATEMENT OF WORK This contract shall provide for Health Physics Radiation Protection Services for Naval Hospitals Camp Lejeune and Cherry Point, North Carolina for Department of Defense owned equipment as outlined in this Statement of Work and as listed on the DD Form 1155 ?Order for Supplies or Services. Radiation Safety Responsibilities An Authorized User (Naval Hospital Staff Radiologist) is to be listed as the Radiation Safety Officer (RSO) on the Naval Radioactive Material Permit (NRMP), and is therefore ultimately responsible for implementation and management of the Naval Radioactive Materials Permit (NRMP) Program. This includes, but is not limited to the responsibility of reviewing all applicable procedures and documentation pertaining to the NRMP Program, and signing the Nuclear Medicine log on a weekly/monthly/quarterly basis, as required. The RSO must also review and sign all radioactive materials shipments. A service contract to carry out the duties of the NRMP will be implemented. The contractor conducting the following duties must have qualifications in accordance with Part 35.50 of Title 10 Code of Federal Regulations (10 CFR 35.50), which can be assessed on the Nuclear Regulatory Commission?s (NRC?s) website at http://www.nrc.gov/reading-rm/doc-collections/cfr/part035/part035-0050.html. Alternate requirements for an ?experienced? RSO can be assessed at http://www.nrc.gov/reading-rm/doc-collections/cfr/part035/part035-0057.html. 1. Review and oversight of Nuclear Medicine Technologists? duties, which include dose calibrator quality control (linearity, accuracy and geometric variation), area radiological surveys, and sealed source inventories and leak tests. 2. Conduct required audits and reviews, which include a semi-annual Radiation Health Audit (Radiation Protection Program) and annual audits of the Naval Radioactive Material Permit and the High Confidence Program (formerly the Quality Management Program). This includes a complete review and audit of Nuclear Medicine records, survey data, ventilation measurements, dose receipt, assay and disposal records, and personnel monitoring. 3. Ensure training is conducted, as required with NUREG 1556, Volume 9, and the NRMP and application, for the following personnel consisting of approximately: 50 Ancillary Staff (i.e. housekeeping maintenance, mailroom and supply receiving), 5 Staff Radiologist (Authorized Users), 25 Nursing Staff (i.e., specific hazards associated with Nuclear Medicine; 45 X-ray) 8 Veterinary Technicians (X-ray Safety), 20 Emergency Department Personnel (i.e., Radiation Accident), Radiation Safety Officer Training, Emergency Medical Personnel (147 Firefighters) and 45 Officers of the Day (Command Duty Representatives). 4. Attendance at quarterly Radiation Safety Committee meetings in an advisory capacity. 5. Conduct an annual Nuclear Medicine Facility Survey to assess Public Dose Compliance in accordance with 10 CFR 20.1301. 6. Perform sealed source leak test for two APD 2000 units and provide oversight/review for sealed source leak tests completed by Nuclear Medicine Technologists. 7. Provide general surveillance of all activities involving radioactive materials and ensuring compliance with the regulations of the U.S. Nuclear Regulatory Commission and special conditions of the license. 8. Review monitoring of all incoming/out-coming radioactive materials for leaks or other conditions constituting a hazard. 9. Review the quarterly inventory of all radioactive materials, which is completed by the Nuclear Medicine Technologist. 10. Comply with all applicable Nuclear Regulatory Commission (NRC) regulations and the Navy Low Level Radioactive Material Disposal Program in coordinating wastes disposal programs, records, and monitoring surveys. 11. Review periodic surveys of all radioactive material usage areas, keeping records of the surveys and informing investigators, as soon as possible, when any radioactivity more than 2 or 3 times above background is detected in hoods or on work surfaces, so that the proper corrective procedures can be undertaken. 12. Provide oversight and guidance for decontamination in accidents involving radioactive materials. 13. Consult with any personnel on all aspects of radiation safety and proper procedure for use of radioactive materials. 14. Provide oversight and advice on the safe handling, monitoring, use, storage and disposal of all radioactive sources associated with the Naval Radioactive Material Permit and that all permitted material is properly secured. 15. Provide oversight and advice to the appointed RSO concerning investigations of overexposures, accidents, spills, losses, thefts, unauthorized receipts, uses, transfers, disposals, medical events or other deviations from approved radiation safety practices and provide guidance for corrective actions as necessary. Radiation Health Responsibilities 1. Conduct an annual review of all exposures to ionizing radiation from X-ray equipment sources to ensure Public Dose Compliance in accordance with 10 CFR 20.1301. 2. Provide oversight to the Command?s Radiation Protection Program, as require by NAVMED P-5055, Title 10 CFR Parts 19 and 20. 3. Conduct semi-annual audits of the Radiation Health Program (Radiation Health Program). 4. Perform Fetal Dose Estimates from Nuclear Medicine or X-ray and CT procedures, as needed. 5. Investigate all infrequent, abnormal, or emergent radiological conditions to identify potential impact on workers and the general public and implement appropriate corrective actions as necessary. Perform Personnel Dosimetry Reviews and Investigations & Dose Estimates, as needed. 6. Review and ensure documentation is maintained to demonstrate that individuals are not likely to receive, in one year, a radiation dose in excess of 10% of the allowable limits or that personnel monitoring devices are provided; and that personnel monitoring devices, when necessary, are used and exchanged at the proper intervals, and records of the results of such monitoring are maintained. Health Physics Responsibilities 1. Provide assistance to this facility in complying with Mammography Quality Standards Act (MQSA) regulations and American College of Radiology (ACR) guidelines by thoroughly evaluating all mammography equipment. This includes 2 units at Naval Hospital, Camp Lejeune, NC and 2 units at Naval Hospital, Cherry Point, NC. 2. Conduct evaluation of the calibration and performance of all types of X-ray equipment, including radiographic units, fluoroscopic units, mammography units, digital systems, mobile units, and CT systems, to include units below: NOTE: The contractor must be board certified by the American Board of Radiology in Diagnostic Radiologic and additionally qualified by the state or by the FDA to performs surveys of mammography units. Documentation must include certificates, as well as documentation of continuing education and experience required. All surveys must meet the requirements of NEHC-TM 6470.03-1 Navy Radiological Systems Performance Evaluation Manual . Conduct evaluation surveys for acceptance of new x-ray equipment or following any servicing or relocation of the equipment. 5. Provide the following services when surveys of x-ray equipment are performed: Technique charts, skin entrance exposures, and regulated parameters (state, FDA, MQSA, and JCAHO,), as well as, parameters essential to good quality control are assesses and documented. Prospective contractors are required to meet the qualifications in accordance with Part 35.50 of Title 10 Code of Federal Regulations (10 CFR 35.50) which can be assessed on the Nuclear Regulatory Commission?s (NRC?s) website at http://www.nrc.gov/reading-rm/doc-collections/cfr/part035/part035-0050.html.
