- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W81K00-06-T-0068 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This acquisition is full and open under NAICS c ode 423450. employees. The requirements in this solicitation are: CLIN 0001, 665 each 5ml Crosseal Kit, Product Code 1935, Mfg: Omrix Biopharmaceuticals, and CLIN 0002, 112 2ml Crosseal Kit, product code1929, Mfg: Omrix Biopharmaceuticals. Federal Acq uisition Regulation (FAR) Clause 52.211-6 Brand Name or Equal applies to this acquisition. Clause 52.211-6 (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/ . Delivery: Product will be ord ered on an as needed basis. The first shipment should be placed on or about 26 Jan 2006. Shipping shall be FOB destination to Brooke Army Medical Center, 3851 Roger Brooke, Receiving Dock, Fort Sam Houston, Texas 78234-6200. FAR provision 52.212-1 [ Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be ac cessed via the internet at www.ccr.gov/ .Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Technical Capability (compliance w ith the requirements of FAR Clause 52.211-6 if vendor offers and Equal Product), past performance, and Price, with Technical Capability being significantly more important than Price. Salient characteristics of product: (1) Preparation time in less than one minutes after thawing, (2) No needle design, (3) Tip is clot free resulting in less waste, (4) All human protein, bovine free, and (5) Thaws within 10 minutes at 37 degree C. Past performance questionnaire: The Great Plains Regional Contracting Offi ce is conducting a competitive acquisition. A past performance questionnaire must be completed by two (2) firms where you have provided similar type of products. Have the questionnaire sent directly back to this office by the point of contact (POC) who c ompleted the form. The firm may fax the completed questionnaire to 210 916-3400 or 4698 (ATTN: Willie Copeland). The POC shall include contact information with the returned questionnaire to include company name, address, and phone number. If an offeror submit more than two (2) past performance references, the Contracting Officer will consider only the first two listed references in the award decision. The Contracting Officer may also consider any additional past performance information that is availabl e to the Government. Offerors are reminded that while the Government may elect to consider data obtained from other sources, the burden of providing thorough and complete performance information rests with the offeror.
