Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. NOTE: All quotes must be emailed to joyce.amador@monterey.army.mil or faxed to 831-242-6591. Solicitation No. W9124N-06-T-0003 is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-15. The associated North American Industrial Classification S ystem (NAICS) code for this procurement is 561320 with a small business size standard of $6.0 Million. The Army Community Services, Seaside CA, requires services of an executive secretary. CLIN 0001, Executive Secretary for the period 1 February 2006 thr u 29 September 2006 in accordance with the Statement of Work indicated below. Quantity: 1352 Hours, Unit Price: $_______ Per Hour. Statement of Work for Army Family Action Plans & Family Programs: 1. GENERAL. 1.1 Contractor shall assist the Government in accomplishing the Army Family Action Plan (AFAP) objectives plus assist in implementing other Army Community Service (ACS) programs that include but are not limited to Family Advocacy (Celebrating Military Children), Military Spouses Day, Armed Forces Tea m Building Recognition Day, Installation Volunteer Ceremony. The Regulation 608-1 entitled Army Community Service Center is located at ACS, Building 4260, Ord Military Community, Seaside CA. 2. Background Information. The mission of the ACS programs is to educate, coordinate and deliver services, which promotes self-reliance, resiliency, readiness and stability within military families. 2.2. ACS will provide four (4) hours of orientation on the focus of each of the above programs at no cost to the Contract or. Training will produce a working knowledge of the local AFAP process and the goals of the above listed programs. 2.3. Location of Work. The services will be performed during the normal duty hours 8:00 AM to 5:00 PM, Monday thru Friday, Federal holidays excepted, at the Army Community Service Office, Building 4260, Ord Military Community. Due to the nature of the services only 10% of the work can be conducted at an independent location. Certain special programs/events will be offered after duty hours and located in government facilities. 3. PERSONNEL. 3.1. All information provided to Contractor by the Government or generated by Contractor in the performance of this contract shall become the property of the Government. This property shall be turned over to the Director, Army Community Services at the expiration of the contract. 3.2. Contractor is required to be able to lift, catch, bend, stoop, reach, and have the ability to climb stairs. 3.3. Contractor is expected to work independently, without direct gov ernment supervision. However, coordination with the Contracting Officer or his designee is considered essential to the successful completion of this contract. 3.4. Failure of the Contractor to provide services stipulated will cause a reduction in payment. 3.5. For payment purpose, Contractor shall submit a copy of his/her monthly invoice to the Director of Army Community or his/her representative for certification of hours worked. For verification of services performed, Contractor shall be required to sign- in prior to starting work. The sign-in sheet will be located at the ACS Office. If Contractor cannot report to work due to illness or personal reasons, he/she will call the ACS Director one hour after the start of normal duty hours. 3.6. Employee Identific ation. Contractor must be recognizable as such on the work site. This shall be accomplished by wearing a name badge/tag on the exterior clothing. The name badge/tag must contain the contractors name and title. It will be the responsibility of the c ontractor to furnish his/her own name badge/tag. 3.7. Qualifications of Contractor Personnel. Contractor shall possess as a minimum a high school degree. A BS/BA degree or 2-year degree is highly recommended. A copy of the diploma or degree achieved shall be submitted with a copy of the proposal. 4. QUALITY ASSURANCE. 4.1. The Government will monitor the Contractors performance under this contract. Government personnel will conduct unannounced inspections for compliance. Government personnel conducting ins pections include, but are not limited to, the Contracting Officer and the Director of Army Community Service or his/her representative. Should the Contractor not concur with the minutes, he/she shall state, in writing, to the Contracting Officer, any area where he/ she does not concur. 4.2. Contractor will coordinate and execute the ACS programs listed at Paragraph 1.1. Services shall be conducted at an available office space at Army Community Service with 10% of the time spent at an independent location. S ervices will not exceed a total of 40 hours a week, 1352 hours for the contract period. Contractor shall not be required to provide services on those days which are Government Holidays (Veterans Day, Thanksgiving, Christmas, New Years, Martin Luther King, Memorial Day, President Birthday, Washington Birthday, Fourth of July). 5. SPECIFIC SERVICES. 5.1. Armed Forces Action Plan. This program identifies concerns of Americas Military families, determines actions required, and assign agencies that are in the P entagon and Department of the Army to implement action to resolve issues. The AFAP is a grassroots process identifying issues of concern to Americas Armed Forces Family that add or detract from the quality of life for the military. Contractor shall develo p with the ACS Director the Army Family Action Plan (AFAP) Focus Groups, which occurs twice a year. a. Contractor shall assist in soliciting issues of well-being, and concerns that have been solicited from the community to include youth/teens, military fam ily members, Navy/NPS, Marines, Air Force, and Coast Guard. Plus California Medical Detachment, ROTC, DMDC, DFAS. Forums where issues can be solicited are Town Hall Meetings, Commanders Meetings, Parent/Teacher Meetings, Boss Meetings, Family Readiness G roup Meetings. Issues will be typed up plus follow-up will be done for clarification with the government organization that is responsible. This should start at least 90 days before each Focus Group). Manage all aspects of the AFAP issues, include screening initial entries, researching, coordinating with proponent offices, maintaining Issue Update book, tracking the issues progress, and apprising principal leaders. b. Contractor shall assist the Army Community Service in recruiting military family members fr om Paragraph 5.1a, plus representatives from the National Guard, Reserves, Retirees and tenant organizations. c. Assist ACS in the marketing of the AFAP Focus Groups through the community news, flyers, posters, housing newsletter, banners, word of mouth an d other creative means. d. Contractor will design, develop, and implement effective marketing strategies and public information program to recruit and utilize volunteers into the AFAP program. Maintain all Government-required forms for volunteers making su re that volunteers support the AFAP process. e. Establish and maintain with the ACS Director a non-governing advisory group that includes, but is not limited to, office and non-commissioned officer spouses to include senior leader spouses. The advisory gro up collaborates with the Army Community Service Director and Installation Commanders Steering Committee to implement initiatives that support the AFAP program and accreditation standards. Make an after action report within 30 days of the meeting. f. Estab lish and execute local AFAP policy guidance/operating procedures. Time-Line for the AFAP objectives: 1) At least 120 days before the set date of the Focus group (Twice a Year) establish marketing of the event, soliciting of community issues and sign-up of the participants will be executed; 2) at least 50 days from the date of the Focus Group the Issues will be coordinated with proponent offices for recommended re solution; 3) assist ACS in hosting facilitators/delegate training. Contractor should partake in this training and be considered a one of the facilitators for the Focus Group; 4) at least 30 days before the Focus Group assist ACS in developing packets for t he delegates, facilitators, and subject matter experts; 5) within 30 days of the Focus Group a written document listing the issues and recommendations will be given to the ACS Dir/Relocation Director for use on the ACS Web Site; 6) an after action report w ithin 45 days on the Focus Group will be due to the ACS Director. 5.2. Develop and implement an Installation AFAP Issue Update Book that covers the top issues from the Symposium. This will be distributed to the community. Review documents and other verific ation or feedback to the community on status of issues and their progress. This update book will be due by the first July 06. 5.3. Establish an AFAP resource library that includes as a minimum, the most recent HQDA AFAP Conference Brochure, AFAP Installati on Handbook, and the HQDA Issue Update Book. 5.4. Contractor shall provide all appropriate administrative and clerical support necessary for the delivery of services herein. Contractor will develop procedures for the maintenance of all records. These recor ds/files will be subject to Government review at all times during the performance of the contract. The files/records will become the property of the Government and shall be turned over to the Army Community Service Director upon the completion or terminati on of the contract. 6. ACS Celebrating Military Childrens Day, ACS Installation Volunteer Ceremony, Military Spouse Day, AFTB Recognition Day: Contractor shall assist Army Community Service in the execution of these community events to include recruiting volunteers, assisting with publicity, communicating/corresponding with units and outside organizations to achieve the objectives of the event. 7. GOVERNMENT FURNISHED PROPERTY AND SERVICES. 7.1. The Government shall furnish space in the Army Community Ser vice, Building 4260, Stillwell Community Center, California. This includes a desk, chair, computer, typewriter, use of copying machine and all office supplies necessary to comply with the statement of work. 7.2. The Government shall furnish the Department of the Army Draft Accreditation Standards, and resource files necessary to comply with this contract. 7.3. The Government shall furnish postal and installation distribution services for all official items. 7.4. The Government will provide limited supplies and equipment necessary for the performance of this contract. The Government property made available under this contract shall be for use only in the performance of the contract. 7.5. The Government shall not be responsible in any way for damage to the Con tractors supplies, materials, equipment, or the Contractors personal belongings due to fire, theft, accident, or other disaster. The Department of Labor Wage Determination No. 1994-2049, Revision 24 applies. The provision at 52.212-1, Instructions to Offerors, applies to this acquisition, as well as any addenda to the provision. Evaluation procedure will be based on the best value to the Government; experience, past performance and price are of equal importanc e. Offerors shall submit a minimum of three references (name, contact information, and capacity/length of relationship) to support experience and past performance. Offerors shall also include a completed copy of the provision at 52.212-3, Offeror Represent ations and Certifications, with their quote. The clause at 52.212-4, Contract Terms and Conditions, applies to this acquisition, as well as any addenda to the clause.
