Description

The Consolidated Contracting Activity, San Francisco VA Medical Center has a requirement for a Contractor to operate and provide Community Based Outpatient Clinic (CBOC) services for the Department of Veterans Affairs. The facility shall be physically located within the city limits of Eureka, CA. Contract personnel shall provide primary care services and mental health facilities for eligible enrolled VA beneficiaries. Estimated number of VA beneficiaries is 5300. Historically, the CBOC contract has been priced on a per member/per month/ capitated basis. Contract period is date of award for one base year with four option year renewals. The NAICS is 621111; size standard is $8.5M. Interested vendors should reply to the Contracting Officer via email with the subject: CBOC Eureka,by close of business January 27, 2006.
