Description
This is an amendment to combined synopsis/solicitation for the purchase of a Medical Waste Disposal Mail-back Service System. Tasks to be Provided: A. Treatment Facility (1) At the time of contract award, the contractor shall have available or provide a medical waste treatment facility for the disposal of the medical waste. This medical waste treatment facility must meet all federal, state and municipal regulations that apply to the receiving and disposal of medical waste through a ?mail back? service. (2) At the time of contract award and annually the contractor shall provide to the FOHS Project Officer copies of current licensures, certifications or other documentation that demonstrates that the medical waste treatment facility that the contractor is utilizing is in compliance with all federal, state and municipal regulations that apply to the receiving and disposal of medical waste through a ?mail back? service. (3) Through out the life of this contract, the contractor shall ensure that the medical waste treatment facility being utilized is in compliance with all federal, state and municipal regulations that apply to the receiving and disposal of medical waste through a ?mail back? service. B. Supplies (1) The contractor shall provide medical waste collection containers in specified sizes (see Appendix C). Additionally the contractor shall provide basket and locking wall mounted brackets for the specified waste collection containers (see Appendix C). All supplies must meet all federal, state and municipal regulations that apply to the shipment, handling and disposal of medical waste through a ?mail back? service. (2) The contractor shall provide the necessary supplies to return the used/full medical waste collection container/s. These supplies must meet all federal, state and municipal regulations that apply to the shipment, handling and disposal of medical waste through a ?mail back? service. (3) At the time of contract award, the contractor shall provide to the FOHS Project Officer documentation confirming that the medical waste collection containers and medical waste shipping containers meet all federal, state and municipal regulations that apply to the shipment, handling and disposal of medical waste through a ?mail back? service. (4) The contractor shall provide a limited catalog of medical waste containers that are available to FOHS?s OHCs (see appendix B). The items on the catalog can only be modified by the FOHS Project Officer. (5) The contractor shall provide, at contract award, an easy-to-use method for ordering and reordering of supplies that will be at no additional cost to the government. The supplies must be shipped to the requesting OHC within 5 business day of placing the order. C. Customer Service and Training (1) The contractor shall provide training to the OHCs listed in appendix B on the proper use of the contractor?s medical waste mail back program within 30 days of contract award. (2) At contract award, the contractor shall provide a toll free customer service line that has coverage from 9:00 AM - 7:00 PM Eastern, 8:00 AM ? 6:00 PM Central and 6:00 AM - 4:00 PM Pacific to address questions from FOHS staff. D. Invoicing and Account Management (1) Within 14 days of contract award the contractor shall develop a method in which they can create or deactivate accounts within seven days after written notification from the FOHS Project Officer. (2) The contractor shall provide a detailed monthly tracking statement to the FOHS Project Officer. This tracking statement shall list the date the order was placed, the ordering location?s unique FOH site code, the size and the number of medical waste containers ordered by each OHC. (3) The shipment cost of the medical waste collection containers to the OHCs, as well as shipment from the OHCs to the medical waste treatment facility that is being utilized by the contractor shall be included in the cost of the medical waste container. (4) The contractor shall provide documentation to the OHC that the medical waste that they generated was destroyed. The following questions and possible answers will be used when evaluating all proposals submitted in response to this RFP - 100 total possible points. 1. Can the contractor provide a medical wasted treatment facility? (25 points) a. Yes but does not meet federal, state and municipal regulations that apply to the receiving and disposal of medical waste. b. Yes but does not meet state and municipal regulations that apply to the receiving and disposal of medical waste. c. Yes but does not meet municipal regulations that apply to the receiving and disposal of medical waste. d. Yes meets all federal, state and municipal regulations that apply to the receiving and disposal of medical waste. 2. Can the contractor provide medical waste collection containers? (25 points) a. Yes but does not meet federal, state and municipal regulations that apply to the shipment, handling and disposal of medical waste. b. Yes but does not meet state and municipal regulations that apply to the shipment, handling and disposal of medical waste. c. Yes but does not meet municipal regulations that apply to the shipment, handling and disposal of medical waste. d. Yes does meet federal, state and municipal regulations that apply to the shipment, handling and disposal of medical waste. 3. Can the contractor provide training on the proper use of the contractors mail back program to all of the OHCs listed on appendix A? (20 points) a. The contractor doesn?t provide training. b. The contractor provides training only via printed instructions on the return-shipping container. c. The contractor provides training only via a hard copy/paper training manual. d. The contractor provides training only via an electronic method (DVD or website). e. The contractor provides training via methods listed in b, c and d. 4. Can the contractor provide a detailed monthly tracking statement? (20 points) a. The contractor cannot provide a detailed monthly tracking statement. b. The contractor can provide only a summary monthly tracking statement that lists each account number and not the unique FOH site codes. c. The contractor can provide a detailed monthly tracking statement that lists each account number and associated with FOHS?s unique OHC site code, the date of the order, the number and size of containers ordered. 5. Does the contractor have experience at providing similar mail back medical waste programs with Federal health care provider and of the size and scope detail in the Statement of Work? (10 pts) a. The contractor has less than one year experience. b. The contractor has one to five years experience. c. The contractor has five to ten years experience. d. The contractor has greater than ten years experience. PLEASE NOTE: the Statement of Work, site listing of all the Occupational Health Centers to be serviced and the estimated quantity per year is no longer available through this amendment.
