Description
THIS IS A NOTICE OF INTENT, NOT A REQUEST FOR A PROPOSAL. A SOLICITATION DOCUMENT WILL NOT BE ISSUED AND PROPOSALS WILL NOT BE REQUESTED. The National Heart, Lung, and Blood Institute (NHLBI), Consolidated Operations Acquisition Center (COAC) for the National Institute of Dental and Craniofacial Research (NIDCR) intends to negotiate and award a purchase order on a noncompetitive basis to Ms. Sherri Gollins, 13311 Tivoli Fountain CT, Germantown, Maryland 20874, to continue to provide the Clinical Research Core (CRC) Dental Hygiene Consulting services. The CRC, provided limited dental care for patients of the Clinical Center and conducts its own research projects. The position for a Full time Dental Hygienist was eliminated as a permanent position in the year 2000. The decision was made to contract out the position. Through competition Ms. Gollins was issued, an award to conduct the services as an on going Research Dental Hygienist. The sole source determination is based upon the knowledge, skills, expertise that the Contractor, posses in the field of oral hygiene dental and medical care of patient needs. Which consist of the knowledge of research procedures for periodontal studies, maintenance of the clinical research database, and four (4) years of experience working with very immuno-compromised patients of the Clinical Center at NIH. In order for NIDCR to continue to obtain the best possible service an award based on the contractor?s familiarity of the required services should be issued. Therefore, an award to any other source would affect the NIH?s anticipated out come based upon the Contractor?s skills, and would not provide any cost savings to the Government. The statement of Work: A. The contractor shall help develop and maintain the clinical trail?s database CRC projects, and act as associate investigator on selected protocols of the CRC. Review all new protocols of the CRC and determine what information will be collected and stored electronically after consultation with the principal and associate investigators of the protocols. Communicate the needs to Information Technology personnel fo NIDCR and notifies them when changes need to be made. Act as an associate investigator on studies, collects research data from patients enrolled in the CRC protocols, and act as coordinator for various research studies, responsible for all dental hygiene treatment of patients who are subjects of NIH research protocol. B. Provide patient care hygiene services as requested by Senior Staff dentists. (1) Patient education, oral hygiene instruction, and recommendations, (2) Periodontal probing and charting, (3) Debriding and scaling, (4) Dispense rinses, gel, and devices to patients to improved home care. C. Assist the clinic staff in treatment of patients referred to the dental clinic. (1) Take radiographs, (2) Take impressions and pour stone models, (3) Fabricate fluoride trays and bite guards. D. Provide preceptorship/instruction to students of hygiene training programs. E. Quality Assurance for the OP1 Dental Clinic (1) 100% audit of all charts, (2) Provide copies of all dental consultations to the referring medical care team and/or the Principle Investigator.F. Patient Care Coordinator for OP1 Dental Clinic. (1) Assure that all patients treated in the NIDCR Dental Clinic receive treatment recommended by dental staff, (2) Assure that all referring doctors are notified of incomplete treatment, (3) Coordination of dental treatment with medical care. G. Serve as OP1 Dental Clinic?s representative to the Hospital Infection Control committee. H. Provide inpatient dental hygiene education as requested by medical teams. The performance of the contract will be evaluated daily and if the performance considered being substandard in the judgment of Project Officer, it shall be documented, and corrective measures of improvement shall be accomplished as well. Schedule: The Contractor shall schedule appointments for patient care for a maximum of eight (8) hours per day. The contractor shall observe all Federal holidays. The Contractor is required to complete a time sheet and provide a signed invoice for the hours claimed to the Administrative Officer (AO) every two (2) weeks and will be paid on a fifteen-day time line. Services shall be performed in the OP1 Clinic in Building 10. Failure to arrived on time or to follow the schedules or instructions as indicated shall be considered default of the contract. Period of Performance is a minimum of 40 hours per week Monday thru Friday total of 1,640 man hours, February 3, 2006 thru September 30, 2006. The North American Industry Classification (NAICS) Code is 541990 and the Size Standard is $6.0M. The acquisition is being conducted under simplified acquisition and is exempt from the requirements of FAR Part 6, Competition Requirement. This notice of Intent is not a request for competitive proposals. Interested parties may identify their interest and capabilities in response to this synopsis. The determination by the Government not to compete the proposed contract based upon responses to this notice is solely with the discretion of the Government.
