- The VA Medical Center, 1970 Roanoke Blvd., Salem, Virginia is seeking potential sources to provide OFF-SITE ENDOSCOPY SERVICES. Contractor shall furnish all labor, materials, supplies, equipment and qualified personnel to provide Endoscopy Services to veteran beneficiaries. The contracted medical facility shall be located within a fifty (50) mile radius of the Salem, Virginia VAMC. Upon receipt of a written consult from Salem VAMC, the Contractor shall schedule the patient and perform the colonoscopy within 30 days; shall provide post-procedure reports; shall provide follow-up care for minor complications arising from the procedure for a two week period; waiting time shall not exceed thirty minutes; emergency care shall be provided as appropriate. Annual estimated amount: 125-150 patients in the base year; estimated 215/yr. for each option year. The following Current Procedural Terminology (CPT) codes are provided for procedures allowed under this contract: 45355; 45378; 45380; 45383; 45384 and 45385. Facility providing services must be JCAHO approved. The applicable NAICS Code is 621111 with a size standard of $8.5 Million. Contract period will be one year from date of award with two available one-year option periods. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company/provider; business size; socio-economic category, if any; if services are on a Federal Supply Schedule; firm?s experience in providing and capability of expeditiously providing Endoscopy Services and any other pertinent information which demonstrates firm?s ability to meet the above requirement.
