- (i) This is a combined synopsis/solicitation for the procurement of commercial items prepared in accordance with the format in Subpart 12.6 and Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this no tice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation document and incorporated provisions, clauses and addenda are those in effect through FAC 2005-05 dt d 14 Nov 2005 and DFAR Supplement, current to DCN 20051209. (iii) This Request for Quotes (RFQ) number is W91YOU-06-T-0010 and the place of delivery is Bassett Army Community Hospital, Medical Logistics Warehouse, Bldg 3567, Fort Wainwright, AK 99703. NOT E: Delivery location is different from Contracting Office annotated above. Quote shall include FOB Destination shipping costs to be included in the unit price of the individual items. (iv) This solicitation is 100% set-aside for Small Businesses. (v) Th e applicable NAICS is 423450 with a size standard of 500 employees. (vi) All prospective quoters must be actively registered in the Central Contractor Registration (CCR) at www.ccr.gov. (vii) This RFQ has seven (7) line items (CLINS): CLIN 0001 Mammogr aphy Label, Right, Manuf. P/N :59700521B, 1-1/4x3/8, Clear, 1000/RL, 4 RL each; CLIN 0002 Mammography Label, Left, P/N 59700523B 1-1/4 x 3/8, Clear, 1000/RL, 4 RL each; CLIN 0003 Instructional Breast Model, multi-type; P/N MT-4, one each; CLIN 0004 Densito meter Clamshell, P/N QC-348B 4-1/2 Volt, one each; CLIN 0005 Directional Marker, P/N SDM-A1, 100/box, 4 boxes each; CLIN 0006 Spee-D-Date Labels, P/N SDD0106000100 2006 100/day January-December/1 Set, 4 Sets each; and CLIN 0007, Spee-D-Date Labels, P/N SDD 0106000500, 2006 500/Day January-December, 1 Set, 4 sets each. All part numbers referenced above are St John Companies, Inc. ALL CLINS ARE BRAND NAME OR EQUAL. If requested, vendor shall provide samples of labels for comparison and evaluation. (viii) D elivery Information: REQUIRED DELIVERY DATE MUST BE NO LATER THAN 23 JAN 2006. (ix) The government intends to award a purchase order to the responsible, responsive quoter whose quote will be the most advantageous to the Government based on price and produ ct quality.
