Description
This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. as implemented by 25 U.S.C. 1638c). This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. Request for Quotation (RFQ) FA9301-06-T-000. This solicitation is restricted to 100% Small Business concerns. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-06. The associated North American Industry Classification System code is 621210 and the small business size standard is $6.5 million. PRICE SCHEDULE-CONTRACT Periodontist: BASIC YEAR ONE (1), CLIN 0001, Period of Performance 1 January 2006 thru 31 December 2006: 416 Hours Per Year @ $__________; BASIC YEAR TWO (2), CLIN 0002, Period of Performance 1 January 2007 thru 31 December 2007: 416 Hours Per Year @$________________; TOTAL: $__________. INTRODUCTION: This position is located at the 95TH Medical Group, 95th Dental Flight located on Edwards AFB, California. The purpose of the position is to provide Periodontist Services in accordance with the Performance Statement of Work (PWS). AFFARS : 5352.201-9101 OMBUDSMAN (AUG 2005) (a) An ombudsman has been appointed to consider and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. The ombudsman may not serve as an advocate for any party. (b) Before consulting with an ombudsman, concerned parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions.) (c?)
