Description
Optometrist FEDERAL PRISON CAMP MONTGOMERY FY 2006 - FY 2010 ?.## Faith-based and Community-based Organizations Faith-Based and Community-Based organizations have the right to submit offers/bids equally with other organizations for contracts for which they are eligible. "The solicitation will be available on or after January 6, 2006, and will be distributed solely through the General Service's Administration Federal Business Opportunities web site http://www.fedbizopps.gov/. The site provides downloading instructions. All future information about this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition." 1. SUBJECT: The provision of Optometrist services to the English and Spanish speaking inmates at the Federal Prison Camp, Maxwell AFB, Montgomery, Alabama under a non-personal service contract for the purpose of providing optical care & evaluation in a clinical setting. 2. SCHEDULE: The work schedule will normally consist of twenty (20) one hour sessions per month. Visits on nights and weekends may be required to provide emergency services; these visits will be counted towards the overall sessions per month. Offeror should be available for telephone consultation. 3. SUPPLIES: The government will supply all items the contractor will routinely need to perform and fulfill the services under this contract. 4. SCOPE OF SERVICES: Performs routine refractions, eye examinations, and tonometry examinations on the inmates of FPC Montgomery. Provides written documentation on all inmates examined. Prescribes and makes adjustments on eye wear. In case of a ye emergency will see the inmate either at his/her office or come into the institution for the examination. 5. SPECIAL QUALIFICATIONS REQUIRED: Licensed as optometrist in the state of Alabama Doctor of Optometry No special experience needed, however previous experience working with inmates is helpful. 6. INMATE MANAGEMENT: Provisions under this contract will require unsupervised contact with inmates. The contractor will not be responsible for the supervision of inmates. 7. RESTRICTIONS ON CONTRACT WORKERS: The contractor under this non-personal service contract will not be allowed to perform the following duties: a. Draw keys for areas other than primary work area. b. Supervise Inmates c. Supervise BOP Staff d. Requisition, sign for or pick up supplies e. Formulate policy or procedures f. Attend recalls or staff functions 8. INSTITUTION SECURITY: The offeror shall attend an orientation program prior to assuming institution involvement. Reimbursement to the contractor for all time spent in the institution orientation or other specific security related training will be at the contract rate prorated hourly. The contractor agrees to adhere to all regulations prescribed by the Institution and the Federal Bureau of Prisons, and regulations governing the personal conduct of individuals entering the premises of the Federal Prison Camp Montgomery. The offeror must pass a background investigation prior to assuming institution involvement. 9. OTHER MISCELLANEOUS INFORMATION: This is a contractual agreement and is not an appointment to an OPM position. Payment shall be based on the accomplishment of specific results, and the results to be obtained shall be entirely within the contractor's own unsupervised determination. Accomplishment of results will be monitored by the contract monitor. 10. ANTICIPATED PERIOD OF PERFORMANCE: Contract award is anticipated in March 2006 pending the availability fo funds. Should funds not be immediately available, contract award for the entire fiscal year will be postponed until such time as funds become available.
